Objective: To clarify the characteristics and risk factors of children's fracture in mainland China,and to provide reference for the construction of prevention strategies for children's fracture.Methods: 1.Computer and manual retrieval of Weipu database,CBM,China HowNet,Wanfang database,Pubmed,Cochrane,Web of Science and Embase database.2.The quality of the study was evaluated by AHRQ and NOS quality evaluation criteria;the cross-sectional study data were meta-analyzed by Stata 11.0;and the case-control study data were analyzed by qualitative descriptive analysis.Results: Twenty-seven cross-sectional studies were included to analyze the regional differences of gender,age,fracture distribution and etiological characteristics in the epidemiological characteristics of children's fracture according to the grouping of eastern,central and western regions of the mainland of China.Three case-control studies were included to analyze the impact of age on the epidemiological characteristics of children's fracture.1.In the east,middle and West regions,the proportion of gender characteristics was higher than that of women;male children were the highest in the central region,with a combined meta-analysis value of 70%[95%CI(67%,73%)],and female children were the highest in the western region,with a combined meta-analysis value of 34%[95%CI(31%,36%)].2.The age span of children included in this study was divided into five groups according to the age stages of children in China: infancy,early childhood,pre-school age,school age and adolescence,and the age characteristics of children's fracture were described accordingly.Infant and early childhood constitutive ratios were the highest in the western region,with 3%[95%CI(3%,4%)] and 29%[95%CI(28%,30%)];meta-analysis combined values of fracture compositional ratios of pre-school and school-age children were the highest in the eastern region,with 36%[95%CI(32%,40%)] and 36%[95%CI(31%,40%)] and 37%[95%CI(32%,42%)] in the central region.3.The highest proportion of fracture distribution in eastern,central and western regions was upper limb fracture,and the highest proportion of upper limb fracture was in the eastern part of China,reaching 78%[95%CI(66%,89%),];the highest proportion of lower limb fracture was in the central part,30%[95%CI(28%,33%];the highest proportion of skull fracture was in the eastern part,32%[95%CI(20%,44%);and the highest proportion of trunk and pelvis in the western part.4%[95%CI(3%,4%)].In upper limb fractures,humeral fracture and ulnar fracture were the highest in the upper limb fractures,humeral fracture and ulnar fracture were the highest in the east,41%[95%CI(30%,52%)] and 6%[95%CI(1%,11%)];radius fracture was the highest in the west,16%[95%CI(15%,18%)];clavic fracture,ulnar fracture and radius fracture and hand fracture were the highest in the middle,15%[95%CI(13%,17%)],21%[95%CI(18%,23%)];lower limb fracture The composition ratio of middle tibia and fibula fracture was the highest in the central region,which was 16%[95%CI(7%,27%)];in the eastern region,femoral fracture and tibial fracture have the highest composition ratio,which were 6%[95%CI(4%,8%)] and 6%[95%CI(2%,9%)];in the western region,tibia and fibula fracture had the highest composition ratio,which was 15%[95%CI(10%,21%)];in the western region,femoral fracture had the highest composition ratio.13%[95%CI(8%,17%)];patellar fracture had the highest composition ratio in the middle,up to 3%[95%CI(2%,4%)].4.Fall injury was the highest risk factor in the east,middle and West regions,which was 41%[95%CI(13%,63%)],38%[95%CI(13%,63%)] and 55%[95%CI(47%,63%)];vehicle injury was the highest in the middle region,accounting for 25%[95%CI(6%,44%);sports injury,fall injury,animal injury,sprain,injury was the highest in the east,fall injury,crush injury,firearm injury was the highest in the west,cycling injury was the highest.The composition ratio is the same in the East and west.5.Decade changes had no significant effect on gender,age,fracture distribution and etiological characteristics of maxillofacial,calcaneal and pelvic fractures in children.Conclusion: 1.There are similarities and differences in the epidemiological characteristics of children's fractures in different regions.Male children are more likely to suffer from falls as a high-risk factor.Children from pre-school to school age in the eastern region are prone to fracture,which is common in humerus fracture;adolescent children in the central region are prone to fracture,which is common in radius and ulna fracture;infant children in the western region are prone to fracture,male is more common,which is common in the skull.Bone fracture.2.The epidemiological characteristics of maxillofacial fracture,calcaneal fracture and pelvic fracture in children may tend to be stable in different years.
