The Clinical Application Value Of TEG In Monitoring Preoperative Blood Coagulation Status Of Patient With Fractures Around The Knee

Objective: To find the changes of preoperative blood coagulation status in patient with fractures around the knee(distal femur,tibial plateau)by means of thromboelastic diagram examination,so as to provide a reference for the timing of preoperative anticoagulation in patients with fractures around the knee.Methods:This was a prospective study.60cases(AIS?3)of in patients with fractures around the knee in our hospital from October 2017 to December 2018 were collected as the experimental group,and 30 cases of health subjects during the same period were selected as the control group.In the experimental group,patients with DVT were excluded from the lower extremity Doppler ultrasound for one time at admission,and venous blood was extracted at admission,12 hours and 24 hours after admission,respectively,for TEG test.SPSS 22.0 statistical software was used to analyze the data.At the test level of ?=0.05,P<0.05 was statistically significant.Result:Compared with the control group,the abnormal rate of TEG parameters in the experimental group increased(P<0.05).Some of the patients in the experimental group showed hypercoagulability upon admission,and most of the hypercoagulability generally occurred within 24 hours after admission Compared with the control group,the mean values of TEG parameters in the test group decreased in R value and K value,and increased in alpha Angle and MA value(P<0.05).During 12-24 hours,the hypercoagulability gradually decreased,R value and K value gradually increased,and alpha Angle and MA value gradually decreased.Conclusions:Most patients with fractures around the knee show hypercoagulability immediately after admission and show an upward trend within 24 hours after admission.Therefore,patients with fractures around the knee can be treated with anticoagulant therapy upon admission.
