Objective: To observe the changes of DTI,1H-MRS and serum MBP in patients with acute carbon monoxide poisoning,and to explore its relationship with delayed encephalopathy.Methods: The subjects of this study were 60 cases of ACOP patients and 80 cases of health examination of Medical Examination Center in our hospital were selected.1.Venous blood of the elbow was collected from all the subjects on the morning of the second day after admission,and the content of MBP in serum was detected.The above indicators of the three groups,namely the cured group,the delayed encephalopathy group and the control group,were panywise compared,and analyze whether there is statistical difference.Blood samples were collected again on the morning of the second day after the onset of corresponding symptoms or the morning of the second day after re-admission to the hospital to detect the content of MBP in the serum of patients in the delayed encephalopathy group,compared to the delayed encephalopathy group of patients in the acute stage and delayed encephalopathy stage serum MBP content,and analyze whether there is statistical difference.2.DTI and 1H-MRS examinations were performed on all subjects 1-3 days after admission,and basal ganglia was selected as the observation area.The average ADC and FA values of selected areas were measured during DTI scanning.The mean NAA,Cho and Cr values of the selected areas were measured during 1H-MRS examination,and the corresponding ratios were calculated with Cr as the substrate.The ADC?FA?NAA/Cr and Cho/Cr of three groups of subjects were compared to analyze whether there were statistical differences.The above examinations were performed again 3 days before the onset of the corresponding symptoms or 3 days before the re-admission of the patients in the delayed encephalopathy group,compare the differences of the ADC?FA?NAA/Cr and Cho/Cr of the delayed encephalopathy group in different periods,and analyze whether there is statistical difference.SPSS20.0 statistical analysis software package was used to analyze the above data.Results:1.Comparison of serum MBP levels between the three groups during acute poisoning.In the acute stage,the serum MBP content of the two groups of CO poisoning were higher than that in the control group,the difference was statistically significant(P<0.01);The delayed encephalopathy group was higher than that of the cured group,the difference was statistically significant(P<0.01).2.Comparison of serum MBP levels of the delayed encephalopathy group at different times.The serum MBP content of patients with delayed encephalopathy was higher than that in acute stage,the difference was statistically significant(P<0.01).3.Comparison of ADC values and FA values between the three groups during acute poisoning.In the acute stage,the ADC and FA values of the two groups of CO poisoning were lower than those in the control group,and the delayed encephalopathy group was lower than that in the cured group,the difference was statistically significant(P<0.01).4.Comparison of ADC values and FA values of the delayed encephalopathy group at different times.Compared with the acute stage,the ADC and FA value decreased,the difference was statistically significant(P<0.01).5.Comparison of NAA/Cr and Cho/Cr between the three groups during acute poisoning.The NAA/Cr of the two groups of CO poisoning were lower than that in the control group,the delayed encephalopathy group was lower than that in the cured group;The Cho/Cr of the two groups of CO poisoning were higher than that in the control group,the delayed encephalopathy group was higher than that in the cured group,and the difference was statistically significant(P<0.01).6.Comparison of NAA/Cr and Cho/Cr of the delayed encephalopathy group at different times.There was no significant change in the delayed encephalopathy period compared with the acute period of NAA/Cr,the difference is not statistically significant(P=0.09);Cho/Cr higher than acute stage,the difference was statistically significant(P<0.01).Conclusion:1.The serum MBP and Cho/Cr levels of DEACMP patients were increased in the acute phase after carbon monoxide poisoning,while the levels of ADC,FA and NAA/Cr were decreased,which could be used as a predictive indicator for the occurrence of DEACMP in ACOP patients.2.After carbon monoxide poisoning,serum MBP and Cho/Cr levels in DEACMP patients were increased compared with that in acute stage,while serum ADC and FA values were decreased.
