Comparative Study Of RPH And PPH In The Treatment Of Ⅲ～Ⅳ Grade Mixed Hemorrhoids

Objective: To observe the clinical effect of Ruiyun Procedure for Hemorrhoids(RPH)combined with the Milligan-Morgan hemorrhoidectomy(M-M)and Procedure for prolapse and Hemorrhoids(PPH)combined with the Milligan-Morgan hemorrhoidectomy(M-M)in treating Ⅲ~Ⅳ grade mixed hemorrhoids.Methods: 80 cases of hospitalization in our hospital were selected according to the inclusion criteria Ⅲ~Ⅳ grade mixed hemorrhoids patients,were randomly divided into experimental group of 40 cases and 40 cases in the control group.40 patients in the experimental group were treated with the Automatic Elastic thread Ligator for Hemorrhoids(RPH-4)combined with the Milligan-Morgan hemorrhoidectomy,the control group of 40 patients were treated with PPH combined with the Milligan-Morgan hemorrhoidectomy.Comparing the operative time,the number of incision,postoperative complications,safety and relapse of the two groups.The results were analyzed by statistical software SPSS19.0,and two-sided test was conducted with α=0.05 as standard.Results:1.The age,sex,course,stage of internal hemorrhoids and types of external hemorrhoids in two groups were comparable.2.The total effective rate of patients in two groups was 100%,and there were no serious adverse events occurred in both groups.3.In the general operation,the experimental group was significantly superior to the controls in terms of operation time and number of incisions,and there were significant differences(p<0.05).4.In the recent postoperative complications,the experimental group was significantly superior to the control group in the 2nd and 7th day after the wound pain score and the most severe bleeding score within 10 days after the operation(p<0.05).There was no significant difference in the grading of Dysuria between the two groups after 24 hours of operation(p>0.05).5.After 3 months follow-up,there was no significant difference in postoperative anorectal stenosis between the two groups(p>0.05).There was no significant difference in the occurrence of serious complication within 3 months after operation and relapse of the two groups after 6 months of follow-up(p>0.05).Conclusion: RPH combined with M-M to treat mixed hemorrhoids and PPH combined with M-M,not only shorten the operation time,reduce the number of surgical incision,and postoperative pain was lighter,bleeding was less.Compared with PPH,there was no significant difference in the occurrence of serious complications.Therefore,RPH is a safe,effective,minimally invasive and simple treatment method,which is more worthy to be popularized than PPH in the treatment of Ⅲ~Ⅳ grade mixed hemorrhoids.
