Objective:To describe the epidemiological characteristics of measles in Chengguan district Lanzhou city by analyzing the changes of the incidence rate from 2010 to 2017.To explore the characteristics of the measle IgM positive rate and the virus gene type in Rash and fever illness(RFI)by describing the laboratory surveillance results of the measles IgM seromarker and pathogenic tests in RFI in the outpatients from 1 October 2016 to 30 September 2017,in order to provide scientific data to guide the local strategy in control and elimination of measles.Methods : Through the "National Notification Disease Reporting system",according to "present address" and "Onset date",we collected the reporting data of the measles in Lanzhou city,Gansu province.We used the methods of describing epidemiology by using the software to calculate the frequency,composition ratio and incidence rate by year,agegroup,gender,and other population characteristics of measles cases.For the laboratory surveillance,subjects was found in the children outpatient departments in 7 hospitals in Chengguan district.Blood and swallow swab sample were collected within three days after rash appeared.Enzyme linked immune sorbent assay(ELISA)was used to test mealses virus IgM with serum and PCR-fluorescent probe was used to detect the virus gene with swallow swab.Data was double inputted by Epi data3.1 software.We used SPSS 21.0 statistical software to analyze the prevalence of measles IgM and the positive rate of virus by gender,agegroup,residence.Results:A total of 347 measles cases were reported in Chengguan district of Lanzhou from January 1st,2010 to December 31 th,2017.The annual cases were 31,9,0,5,18,195,81 and 8from 2010 to 2017,respectively.The average incidence rate of measles was 3.37/1000000,and the annual rate was 2.41,0.71,0,0.39,1.41,15.2,6.24 and 0.61 per 1000000.In 2010-2017,the incidence of measles under 1 year old was higher than that of other age groups.The reporting incidence of male was 0.61/100,000-17.62/100,000 and was higher than female in most years except 2014.The total reporting case of the 1st,2nd,3rd,4th quarter from 2010 to 2017 was 32(9.22%),41(11.82%),32(9.22%),242cases(69.74%),.68 cases(19.60%)were migrant population 329 case(94.81%)had vaccination history.Among the 622 RFI cases surveyed,384cases(58.01%)were male.The cases among children aged under 8-month,8 month-5 years and6-14 years were 77(11.63%),462(69.79%),123(18.58%)respectively.110 cases(16.62%)were migrant population,and 94.70%(554/585)cases had vaccination history.The total positive rate of the measles IgM was 11.63%(77/662).For the persons under 8 month and 8 months-5 years old,the IgM positive rate was 1.3%(1/77)and 16.5%(76/462).For the male and female,the measles IgM positive rate was 9.9%(9%(38/384)and 10.2(39/278),and for the local resident and migrant population the measles IgM positive rate was 10.9%(60/552)and 15.5%(17/110).Among the 323 RFI cases with swallow swab sample,the total positive rate of measles virus was 18.9%(61/323)and all was the H1 a gene type.For the male and female,measles virus positive rate was 18.40%(33/179)and 19.40%(28/144)respectively,and for the local resident and migrant population the measles virus positive rate was 18.5%(18.5%)and 21%(11/52)respectively,and for the persons under 8 month and 8 months-5 years old,the measles virus positive rate 5.9%(1/17)and 21.3(60/282).Conclusion: The incidence of measles in Chengguan district Lanzhou city Gansu province didn't achieve the goal of measles elimination from 2010 to 2017.The reporting cases are mainly for children less than 1 year old,for the local population,and mainly being reported in winter,and incidence of male was higher than female.IgM positive rate and measles virus positive rate were high among children aged 8months to 5 years old(P<0.05).It was recommended to continue to strengthen the monitoring of measles cases,and to identify and control outbreaks in time.Strengthen hospital infection control and reduce the spread of measles cases in hospitals.We will continue to strengthen the regular immunization of children's measles vaccines,especially for the migrant population,to ensure the high coverage of two measles vaccines.It was also recommended to investigate the risk factors of cases and make the effective control measures.
