The Value Of Copeptin In The Diagnosis And Evaluation For Critical Patients

Objective:To study the level of plasma Copeptin and its clinical significance in the course of acute critical diseases.Methods:(1)One hundred and thirty-six patients presenting into emergency departments with chest chocking or chest pain within 6 hours were enrolled.copeptin and c Tn I was measured.The diagnosis was adjudicated by 2 independent experts using all available data.The blood samples were tested for copeptin and c Tn I at the same time.The diagnostic performance of them was assessed using ROC analysis.The sensitivity and specificity were inferred based on the positive rate of two cardiac markers.(2)Total of 175 patients with acute poisoning were enrolled in this study.The levels of plasma Copeptin were measured by ELISA in all subjects.The relationship of the level of plasma Copeptin in patients with acute poisoning and APACHE?score were analysed.The diagnostic performance of them was assessed using ROC analysis.The sensitivity and specificity were inferred based on the positive rate of two clinical index.(3)Total of 98 patients with Traumatic brain injury were enrolled in this study and divided into three groups according by GCS score.The levels of plasma Copeptin were measured by ELISA in all subjects.The relationship between the level of plasma Copeptin in patients with acute poisoning and GCS score were analysed.The diagnostic performance of copeptin for Prognosiswas assessed using ROC analysis.The sensitivity and specificity were inferred based on the positive rate of two clinical index.(4)One hundred and fourteen patients presenting into emergency departments with multiple injuries were enrolled and divided into minor injuries,serious injuries and severe injuries groups.Copeptin were determined with ELISA in all subjects.The relationship between Copeptin in patients with multiple injuries and ISS score were analysed.The Prognosissignificance of copeptin was assessed by using ROC curve.(5)Fifty-two patients with Sepsis and forty patients with SIRS were enrolled.The levels of plasma Copeptin,CRP,PCT were measured in all subjects.The relationship of the level of plasma Copeptin,CRP,PCT and APACHE?score were analysed.The diagnostic performance of them was assessed by using ROC cueve.Results:The levels of copeptin in critical patients were obviously higher than other groups.And the increased levels of copeptin indicates the serious illness and bad prognosis.The dynamic change of this index is of great value to the condition evaluation and prognosis judgment.Conclusion:Determination of copeptin for critical patients(Myocardial infarction,acute poisoning,craniocerebral trauma,multiple injuries,sepsis)can improves diagnostic performance,and may be an important clinical parameter to predict severity and prognosis,guide therapy in these patients.
