Objective:1.To explore the application value of transrectal contrast-enhanced ultrasonography in identification of the staging of preoperative bladder cancer.2.The correlation between the quantitative parameters of the time-intensity curve of transrectal contrast-enhanced ultrasonography and the postoperative pathological grading of bladder cancer. Methods: Two-dimensional ultrasound and CEUS characteristic of 72 patients(low level 37 cases, high level 35 cases)of bladder cancer confirmed by pathology after resection of bladder tumor between December 2016 and November 2018 in Shengjing hospital of CMU,was analyzed retrospectively.TIC parameters included rise time(RT),peak intensiy(PI),mean transit time(MTT),time from peak to one half(TPH),time to peak(TP),wash in slope(WIS),semi-descending slope(DS)were acquired and CEUS characteristics were analyzed by QLAB software.Analyze the correlation between CEUS quantitative parameters and postoperative pathological grading of bladder cancer.Results:1.In terms of the staging diagnosis of bladder cancer, the overall coincidence rate between the diagnosis result of CEUS in the rectum and the pathological diagnosis was 90.3%.After the consistency test of the diagnostic test,Kappa=0.826,the diagnosis have good consistency(P < 0.05).The sensitivity, specificity and coincidence rate of CEUS were 96.0%,86.6%,90.2%,81.3% and 97.5%.2.In comparison between of CEUS quantitative parameters of TIC curve in low level and high level of bladder cancer, the PI?TPH?TTP and WIS was higher and RT?MTT and DS was lower in high grade group than in low grade group(PI:t=3.42,p=0.001;TPH: t=6.45,P<0.001;DS: t=-9.93,P<0.001),but RT,MTT,TP,WIS were no statistically significant between two groups(all P>0.05).Conclusion:1.Transrectal contrast-enhanced ultrasonography has a high diagnostic efficacy for preoperative bladder cancer staging, and can accurately distinguish invasive from non-invasive urothelial carcinoma, and the results are highly consistent withpathological diagnosis.2.The time-intensity curve quantitative parameters of transrectal contrast-enhanced ultrasonography are of certain value in differentiating the malignant degree of bladder urothelial carcinoma.
