Objective: Lumbar disc herniation(LDH)is a common disease in spinal surgery,and some severe patients may require surgical intervention.In recent years,minimally invasive surgery has gradually become the mainstream for the surgical therapy of LDH,of which transforaminal endoscopic spine system(TESSYS)is one of the classic techniques.Targeted foraminoplasty monitored by spinal endoscopy is an improvement based on the principle of TESSYS technique.The purpose of this study was to clarify the initial clinical efficacy of targeted foraminoplasty monitored by spinal endoscopy in the patients with the lumbar disc herniation at the L4-5 levels,and to compare it with the traditional TESSYS technique.Methods: A retrospective assessment was performed involving 49 cases of LDH from January 2015 to December 2015.31 patients with lumbar disc herniation at the L4-5 levels were managed with the targeted foraminoplasty monitored by spinal endoscopy as the treatment group,while other 18 patients with lumbar disc herniation at the L4-5 levels were treated with TESSYS technique as the control group in the same period.The modified Mac Nab efficacy criteria and ODI score were used to evaluate the postoperative functional recovery,and VAS score was used to compare the clinical effects before and 3 months,6 months and 12 months after the operation.The patients' willingness to recommend this operation was obtained by telephone follow-up.Results: Compared with the control group,the treatment time was significantly shorter(P <0.05).The postoperative VAS and ODI scores of the two groups at 3 months,6 months and 12 months were significantly decreased in comparison with preoperative period(P <0.05);however,the VAS and ODI scores of the two groups at each follow-up point.There was no significant difference between the two groups(P >0.05).Compared with the control group,there was no significant difference in the improved Mac Nab rate and the willingness to recommend the operation after surgery of the treatment group(P >0.05).Conclusion: The targeted foraminoplasty monitored by spinal endoscopy has the same curative effects as the traditional foraminoplasty technology in the treatment of lumbar disc herniation,with the advantages of shorter time-consuming,less radiation,less difficulty to puncture,and easier to adjustment.
