The Clinical Characteristics And Two Different Treatment Outcomes Of 321 Patients With Hypopharyngeal Carcinoma

Objective:To investigate the clinical characteristics,two different treatment outcomes and prognostic factors of hypopharyngeal carcinoma.Methods:A retrospective analysis was performed on the complete clinical and follow-up data of 321 patients with pathologically confirmed hypopharyngeal squamous cell carcinoma at Liaoning Tumour Hospital from 2005.01 to 2016.12.The life table method was used to calculate the overall survival?OS?rates,Log-rank test was used to compare the OS between the two groups,the Cox proportional hazard model was used to perform the multivariate analysis to confirm independent treatment modalities as prognostic factors.Results:Among the 321 patients,27 were lost to follow-up,the patients who were lost to follow-up were treated with death and were counted at the last effective follow-up time.The follow-up rate of 321 patients was 91.59%,and the median and mean follow-up time were 25.47 months and 36.87 months,respectively.Among the 321patients,197?61.4%?received surgery combine with radiotherapy or concurrent chemoradiotherapy treatment?S+R/CRT?and 124?38.6%?received radiotherapy or concurrent chemoradiotherapy treatment?R/CRT?.For 321patients,the 1?3?5-year overall survival?OS?was 75.87%?49.39%?41.38%and the median survival time was37.65 months;the 1?3?5-year overall survival?OS?of 197?61.4%?who received surgery combine with radiotherapy or concurrent chemoradiotherapy treatment?S+R/CRT?was77.68%?58.26%?48.81%;the 1?3?5-year overall survival?OS?of 124?38.6%?who received radiotherapy or concurrent chemoradiotherapy treatment?R/CRT?was72.98%?32.56%?27.69%.Univariate analysis showed clinical stage of tumor??+?compared with?+?,P=0.019?0.05??T stage(T_1-2-2 compared with T_3-4,P=0.014?0.05)?N stage?N₀ compared with N₊,P=0.002?0.05??M stage?M₀ compared with M₁,P=0.029?0.05?and two different treatment modalities?S+R/CRT compared with R/CRT,P=0.001?0.05?have impact on survival prognosis.Cox regression multivariate analysis showed T stage?P=0.010?0.05??N stage?P?0.001??two different treatment modalities?P=0.002?0.05?were independent risk factors of prognosis.Conclusion:The overall prognosis of hypopharyngeal carcinoma was poor and dismal.Hypopharyngeal carcinoma is characterized by high degree of malignancy,difficult to be found early,prone to recurrence and metastasis after operation,large trauma and poor prognosis.Comprehensive examination should be conducted to define the stage of tumor and choose the rational treatment plan before treatment,and surgery combine with radiotherapy or chemotherapy treatment modality is still the main treatment strategy for advanced-stage Hypopharyngeal carcinoma.
