| Objective: To investigate the relationship between changes of serum phosphorus and Fibroblast growth factor-23(fgf-23)and bone metabolism of neonatal with different gestational age and their significance in the pathogenesis of metabolic bone disease.Methods: This study selected 35 neonates as research objects,who were admitted to the neonatal intensive care unit of the second hospital of lanzhou university from March 1,2018 to December 31,2018.All the neonates in the study were divided into full-term infant group,early premature infant group and late premature infant group according to their gestational age.All newborns in the study were divided into normal weight group,low birth weight group and very low birth weight group according to body weight.All neonates in the study were divided into the use group and the non-use group according to whether they were given ventilator support or medication(such as hormones,diuretics,methylxanthine,theophylline,etc.).Blood biochemical examination and bone metabolism were completed within one week after birth,and serum samples were collected for fgf-23 detection.After data collection,SPSS22.0 software was used for statistical processing and analysis.Results: 1.The univariate analysis of variance indicated that the gestational age,body weight and blood phosphorus of the premature infants were lower than those of the full-term infants,with the early premature the lowest.The gestational age,body weight and blood phosphorus of the low birth weight infants were lower than those of the normal weight infants,with the very low birth weight the lowest(P<0.01).2.The univariate analysis of variance suggested thatThe parathyroid hormone of premature infants was higher than that of full-term infants,among which the parathyroid hormone of early premature infants was the highest,the parathyroid hormone of low birth-weight infants was lower than that of normal birth-weight infants,and theparathyroid hormone of extremely low birth-weight infants was the lowest(P<0.01).3.The univariate anova suggested that fgf-23 in the premature infant group was lower than that in the full-term infant group,and the fgf-23 in the early premature infant group was the lowest(P<0.05).4.The data of rank sum test indicated that the alkaline phosphatase levels in the premature infant group and the low-birth-weight infant group were higher than those in the full-term infant group and the normal-birth-weight infant group,respectively,and the early premature infant group and the extremely low-birth-weight infant group were the highest(P<0.01).5.The independent samplet test analysis data indicated that parathyroid hormone and alkaline phosphatase were higher in the group using drugs or in the respiratory group than in the group not using drugs(P<0.05).6.The pearson correlation analysis showed that neonatal gestational age was positively correlated with serum phosphorus and fgf-23(r=0.506,0.477,p<0.01),and negatively correlated with parathyroid hormone and alkaline phosphatase(r=-0.628,-0.576,p<0.01).Neonatal body weight was positively correlated with gestational age,serum calcium,serum phosphorus,and fgf-23(r=0.905,0.360,0.585,and 0.490,p<0.05),and negatively correlated with25(OH)D3,parathyroid hormone,and alkaline phosphatase(r=-0.352,-0.542,and-0.585,p<0.05).Conclusion:1.The younger the gestational age of the newborn,the lower the body weight,the lower the blood phosphorus,the higher the alkaline phosphatase.2.The younger the gestational age of the newborn,the lower the serum fgf-23. |
