OBJECTIVE: To understand the characteristics of spino-pelvic sagittal parameters in patients with lumbar disc herniation.To explore the influence of different gender and age on the spino-pelvic sagittal parameters.To evaluate the correlation between spino-pelvic sagittal parameters and preoperative quality of life in patients with lumbar disc herniation.To explore whether PELD technology can improve spino-pelvic sagittal parameters in patients with lumbar disc herniation.METHODS: 1.Retrospective analysis of 120 patients with lumbar disc herniation from January 2016 to January 2019 in our department of orthopedic surgery.The spino-pelvic sagittal parameters were measured on the full-length standing positive lateral radiograph of the spine and pelvis(TK,LL,PI,PT,SS,PI-LL,SVA).Collect statistical ODI values.The patient’s spino-pelvic sagittal parameters were compared according to gender and age group.The correlation between preoperative spino-pelvic sagittal parameters and ODI values was analyzed.2.Retrospective analysis of patients who underwent PELD for lumbar disc herniation from January 2016 to February 2019 in our hospital.The full-length standing positive lateral radiograph of the spine and pelvis were taken before and at the last follow-up,and the measurements were obtained.The patient’s spino-pelvic sagittal parameters were compared.RESULTS: There were no significant differences between the spino-pelvic sagittal parameters(TK,LL,SVA,PI,PT,SS,PI-LL)in male and female patients with lumbar disc herniation.There were no significant differences in spino-pelvic sagittal parameters(TK,LL,SVA,PI,PT,SS,PI-LL)between the two groups of patients <45 years old and age ≥45 years.A comparison of ODI values between men and women found that the male ODI value was 46.94±20.09,and the female ODI value was 55.92±21.26,P=0.020(P<0.05),the difference was statistically significant.There is no correlation between the spino-pelvic sagittal parameters(TK,LL,SVA,PI,PT,SS,PI-LL)and healthy quality of life in patients with lumbar disc herniation.After treatment of lumbar disc herniation with PELD technique LL was significantly improved after operation(34.66±10.04° before operation,38.93±9.06° after operation,P<0.05).The difference was statistically significant.Conclusion:This study did not find that the spino-pelvic sagittal parameters of patients with lumbar disc herniation were affected by gender and age.There was no difference in PI between men and women(P=0.051,P>0.05),but the PI of women with lumbar disc herniation has a significantly larger trend.Further studies were needed to increase the sample size and age difference.Preoperative healthy quality of life in patients with lumbar disc herniation is not affected by the spino-pelvic sagittal parameters.At the same time,this study found that female patients with lumbar disc herniation have a worse quality of life than men,and combined with the consideration of whether there is a difference in PI,further research is needed.Patients with lumbar disc herniation had significant improvement in LL after PELD treatment,but no significant changes were found in SVA,TK,PI,PT and SS.
