Objectives:1.Comparison of the advantages of multi-slice spiral CT post-processing reconstruction technique and conventional CT scanning technique in differential diagnosis of solitary pulmonary nodules(SPN)on display rate of image signs and diagnostic accuracy,sensitivity,specificity,positive predictive value and negative predictive value.To provide more powerful guarantee for accurate clinical diagnosis and treatment of SPN2.To analyze the benign and malignant risk factors of SPN using multi-slice spiral CT post-processing reconstruction techniques,and to provide strong evidence for clinicians to better manage SPN patients.Methods:1.63 patients with SPN were enrolled and underwent the whole chest volume scanning using multi-slice spiral CT(MSCT).The data was reconstructed by a standard reconstruction algorithm with a layer thickness of 0.625 mm,then the data was transmitted to CT post-processing workstation provided by GE Company for multi-planar reconstruction(MPR),curved planar reconstruction(CPR),maximum density projection(MIP),volume rendering(VR)and other post-processing.Display rate of main image signs such as spicule sign,lobulation sign,pleural traction sign,vascular-bronchial cluster sign,vacuolar sign and air bronchial sign using post-processing reconstruction technique and conventional CT scanning technique was respectively compared.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of the two methods in differential diagnosis SPN were compared.The count data was expressed by percentage(%),and Chi-square test was used to compare the rates between groups,SPSS 21.0 was used for statistical analysis.P < 0.05 indicates statistical difference.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of the two methods in differential diagnosis SPN were calculated using Medcalc software(11.4.2.0).2.A retrospective analysis of 97 patients with SPN who underwent surgical resection and pathological diagnosis in the Affiliated Hospital of Air Force Medical University from November 2015 to September 2019 was performed.The following factors that may affect the benign and malignant judgement of SPN were collected:(1)general clinical situation and medical history: age,gender,previous tumor history,family tumor history,history of COPD,history of tuberculosis;(2)imaging signs of SPN using CT post-processing technique: location,maximum diameter of lesion,neoplasm-lung interface,calcification composition,ground glass composition,lobulation sign,spicule sign,pleural traction sign,vascular-bronchial cluster sign,vacuolar sign and air bronchial sign.The above 18 items were first analyzed by single factor analysis,and then by multivariate logistic regression analysis if there were statistical significance.Results:1.For the SPN spicule sign,lobulation sign,pleural traction sign,vascular-bronchial cluster sign,vacuolar sign and air bronchial sign,the display rate of the CT postprocessing reconstruction group was significantly higher than that of the conventional CT group.2.The diagnosis accuracy,sensitivity,specificity,positive predictive value and negative predictive value of SPN using post-processing reconstruction technique were higher than those of the conventional CT scanning.3.Single factor analysis showed that nodule location,maximum diameter,neoplasm-lung interface,calcification composition,ground glass composition,lobulation sign,and pleural traction were helpful in the diagnosis of benign and malignant SPN.4.Multivariate logistic regression analysis showed that maximum diameter of nodule and ground glass composition were independent risk factors of malignant SPN..Conclusions:1.The CT post-processing reconstruction technique has obvious advantages over conventional CT scanning techniques in displaying the main imaging signs of SPN such as spicule sign,lobulation sign,pleural traction sign,vascular-bronchial cluster sign,vacuolar sign and air bronchial sign.2.The diagnosis accuracy,sensitivity,specificity,positive predictive value and negative predictive value of SPN using CT post-processing reconstruction technique were higher than those of the conventional CT scanning.3.In the differential diagnosis of benign and malignant SPN,nodule location,maximum diameter,neoplasm-lung interface,calcification composition,ground glass composition,lobulation sign,and pleural traction sign should be observed carefully.4.The maximum diameter of nodule and ground glass composition are independent risk factors of malignant SPN...
