Objective To analyze the clinical features of gallbladder carcinoma,to summarize the surgical treatment experience and the prognostic factors affecting the survival of patients with gallbladder cancer.Methods The retrospective cross-sectional study was adopted,data of 150 patients with gallbladder cancer in our hospita from January 1,2008 to December 31,2012 were collected.The patients were followed up by outpatient examination or telephone.For the follow-up endpoint,the deadline was December31,2018.The univariate analysis showed that(age,gender),clinical case characteristics(gallbladder stones,preoperative jaundice,surgical methods,pathological type,tumor differentiation,tumor stage(TNM stage and Nevin stage),liver infiltration,lymph node metastasis and distant metastasis.The count data was expressed as a percentage,the Kaplan-Meier method was used to plot the survival curve,the univariate analysis and multivariate analysis was performed using the Log-rank test and COX regression model.Results Of 150 patients,114 were followed-up,for a follow-up 120 months.With the average survival time was 26.4 months and median survival time was 9 months.The 1-,3-,5-year survival rates of 45.6%,22.8% and 16.7%,respectively.In the univariate analysis revealed that preoperative jaundice,liver infiltration,lymph node metastasis,distant metastasis,surgical methods,pathological grading,tumor stage(TNM staging and Nevin staging)were risk factors for prognosis.In the multivariate analysis showed that surgical methods(simple cholecystectomy and gallbladder cancer extended radical surgery)and tumor differentiation(low differentiation)were independent risk factors for survival time in patients with gallbladder cancer.Conclusion:1.Gallbladder cancer clinical manifestations are not typical,the incidence of occultation,leading to the majority of patients in the advanced stage.2.There was no significant correlation between gender,age,gallbladder stones,pathological type and the prognosis of patients with gallbladder cancer.3.Preoperative jaundice,liver infiltration,lymph node metastasis,distant metastasis,Nevin stage,TNM stage,pathological grading and surgical methods were significantly correlated with the prognosis of patients with gallbladder cancer.4.Pathological grading(low differentiation)and surgical method(simple cholecystectomy,gallbladder cancer extended radical surgery)is an independent risk factor for the prognosis of patients with gallbladder cancer.5.Radical cholecystectomy has a better prognosis in the surgical methods of gallbladder cancer.
