Objective To Analysis of risk factors of spontaneous bacterial peritonitis associated with ascites in patients with hepatitis B cirrhosis.Methods Retrospective analysis of 168 cases of hepatitis B cirrhosis ascites with Spontaneous bacterial peritonitis hospitalized in our hospital from January 2015 to August 2018,and132 cases of ascites from hepatitis B cirrhosis in the same period.The clinical data of the two groups were compared by SPSS20.0 statistical software.The single factor statistically significant data were used for multivariate logistic regression analysis to obtain the independent risk factors of ascites of hepatitis B cirrhosis with spontaneous bacterial peritonitis.Results 1.According to the inclusion of diagnostic criteria,a total of 300 subjects were selected.There were 132 cases of hepatitis B cirrhosis with ascites group,88 males and 44 females,male: female ratio was 2:1;hepatitis B cirrhosis ascites and spontaneous bacterial peritonitis group 168 cases,male 128 cases,40 women,male: female ratio was 3.2:1.2.The results of single factor analysis was showed:Age(P=0.001),White blood cell count(P=0.024),Total bilirubin(P=0.001),Serum albumin(P=0.044),Proth-rombin time(P=0.023),Prothrombin activity(P=0.037),International standard prothrombin time(P=0.004),Portal vein width(P=0.038),Ascites total protein(P=0.019),Ascites lactate dehydrogena-se(P=0.047)),Ascites multinucleated cell count(P < 0.001),MELD score(P =0.008),Child-Pugh grade(P = 0.011),Liver failure(P = 0.040)and Hepatic encephalopathy(P = 0.016)were statistically significant(p<0.05);Gender(P=0.068),History of diabetes(P=0.610),Hemoglobin(P=0.977),Platelets(P=0.267),Aspartate aminotransfer-ase(P=0.060),Alanine amin otransferase(P=0.052),Alkaline phosphatase(P=0.754),r-gl utamyltransferase(P=0.402),Serum lactate dehydrogenase(P=0.115),Ascites(P=0.729),Spleen thickness(P=0.308),Gastrointestinal bleeding(P=0.858),Gallstones(P=0.277),and Electrolyte disturbance(P=0.872)were not statistically significant(p>0.05).3.The results of multivariate logistic regression analysis of meaningful single factors were shown:Ascites PMN(OR=2.535?95%CI 7.894-11.510?P=0.000),Ascites total protein(OR=0.326?95%CI 0.157-0.677?P=0.003)and Child-Pugh classification(OR=9.110?95%CI1.053-8.797?P=0.045)were statistically significant(p < 0.05).Conclusion Ascites PMN,Ascites total protein and Child-Classifica-tion may be independentrisk factor for hepatitis B cirrhosis ascites and spontaneous bacterial-peritonitis.
