Investigation On The Periodontal Health Status And Oral Cognitive Behavior In Rheumatoid Arthritis Patients

Objective:To study the periodontal health status and oral cognitive behavior of patients with rheumatoid arthritis(RA),to explore the relationship between the severity of periodontitis and RA,and to provide suggestions for oral health care of RA patients.Methods: According to the method of random sampling,135 hospitalized patients in the Department of Nephropathy and Rheumatism,Affiliated Hospital of Zunyi Medical University from March 2018 to October 2018 were selected as RA group,130 healthy people in the same period were selected as control group.Oral periodontal examination was performed in the RA group and the control group.The RA group was investigated for rheumatoid-related examination and oral cognitive behavior,and the periodontal status and severity of the RA group and the control group were compared.Discussion RA group oral cognitive behavior,combined with the actual situation,put forward to the oral health care measures of RA patients?Results:(1)(1)The detection rate of periodontitis in RA group was higher than that in control group(P<0.05).(2)Compared with the control group,there was no significant difference in the number of missing teeth due to periodontitis and the positive rate of BOP between the RA group and the control group(P> 0.05).There were significant differences in PLI,PPD and CAL between the two groups(P< 0.05).(3)There were significant differences in the number of periodontitis patients between RA group and control group(P< 0.05);(4)The titers of CRP,ESR and anti-CCP antibodies in RA group were significantly different in different degrees of periodontitis(P< 0.05).The activity level of the three groups increased with the severity of periodontitis.(5)ESR was significantly correlated with CAL,BOP and PDI(P=0.002,0.020,0.002),but not with PPD and PLI;CRP was correlated with PLI,CAL,BOP and PDI(P=0.046,0.000,0.005,0.002),but not with PPD;anti-CCP antibody titers were correlated with CAL,BOP and PDI(P<0.05),but not with PPD and PLI;Morning stiffness duration was not correlated with periodontal parameters(P>0.05)?(2)(1)In the statistics of oral cognitive behavior in RA group,47.41% of the patients considered oral health important;36.30% of the patients thought that the bleeding of brushing teeth was normal;only 33.33% thought that gingival bleeding was caused by gingival inflammation;and those who thought that not cleaned teeth was more beneficial than cleaned teeth(44.44% vs 40.75%);(2)and the frequency of brushing teeth in RA patients was mostly twice a day(72.59%);Most of the patients brushed their teeth for 1 minute(37.78%);about 53.33% never used floss.(3)In the history of oral periodontal condition,49.63% of the RA patients indicated brushing or bleeding bites;31.11% of the RA patients indicated loose teeth or weak chewing;39.26% of the RA patients had bad odor and 26.67% had periodontal swelling and pain.(4)In the history of oral periodontal therapy,the number of patients who never washed their teeth was more than that who washed their teeth regularly(47.41% vs 26.67%).Only 31.11% of the population would check oral hygiene regularly,and about 36.67% of the population had received professional oral hygiene guidance.(5)The tendency of the publicity of oral health knowledge of the patients with RA preferred imaging products(film,animation),pictures and materials and organized oral health and related activities.Conclusion:1.RA may be more susceptible to periodontitis,and the severity of periodontitis is more serious.ESR,CRP and anti-CCP antibodies were significantly correlated with CAL,BOP and PDI,suggesting that the two diseases may be related to some extent.2.Oral cognitive behavior and oral hygiene habits of RA patients are generally poor.Oral hygiene education and periodic periodontal health examination should be strengthened for RA patients.
