A Study On Psychological Status Of Breast Cancer Patients

Objectives: To explore the psychological status of breast cancer patients at different stages and its related factors in depth.Methods: During May-August 2017,psychosocial questionnaires are sent to long-term breast cancer patients attending our hospital.The questionnaires include Patients' Health Questionnaire Depression Scale-9 item,Generalized Anxiety Disorder Scale-7 item,Body Image after Breast Cancer Questionnaire-Chinese version,Functional Assessment of Cancer Therapy-Breast,Social Support Rating Scale.,respectively,which focus on these four aspects include general anxiety and depression,appearance and sexual attraction,quality of life and social support.And then we add up total scores on each scale.We use statistical methods to compare different psychological status of breast cancer patients in different periods and the differences of the overall psychological status,so that we can understand focus of the mental issues in each period,and further explore the related factors of psychological status of patients with breast cancer.Results: The proportion of depression and anxiety among 173 breast cancer patients was 39.9% and 38.7%.173 subjects are divided into depression group and non-depression group.The result shows that there is a positive relationship between depression and body-image evaluation,a positive relationship between depression and social support,a negative relationship between depression and anxiety.173 subjects are divided into six groups according to how long it has been when diagnosed.There is no statistical difference in social-population data and tumor-related data among the groups.The results show that there is no difference in depression in breast cancer patients in the first three years.After three years,the incidence of depression is significantly decreased(Sig value < 0.001).There is no difference in the anxiety of breast cancer patients in the first four years,and the incidence of depression declines significantly after four years(Sig value=0.003<0.05).There is no difference in the quality of life and self-body evaluation of breast cancer patients in the first five years.After five years,the quality of life and self-body evaluation significantly improve(P =0.011 <0.05,P < 0.001).There is no statistically significant difference in the social support of breast cancer patients among the groups(P = 0.100> 0.05).We choose quality of life as the dependent variable,choose depression,anxiety,body image,and social support as independent variables,and use multiple linear regression analysis.The R value is 0.754,the R2 value is 0.569,and the adjusted R2 value is 0.559,which explaining the 56.9% variation in quality of life.There is a medium intensity correlation.The F test shows that the Sig value is <0.001,which means statistically significant.Conclusions: Although the depression,anxiety,quality of life,and body image assessment of breast cancer patients improves over time,they are still significantly different from the normal.Especially in the first four or five years of diagnosis,the negative psychological mood of breast cancer patients needs more attention.The majority of breast cancer patients in clinic can not get professional psychological treatments.This requires the breast surgeon to pay attention to the patient's psychological mood in the clinical treatment process,and to master the necessary psychological intervention measures to help breast cancer patients through the period of serious negative psychological emotions.With the development of medical technology,the survival rate of breast cancer patients is greatly improved and the survival time is greatly extended.Therefore,the quality of life is particularly important for breast cancer patients.Depression and anxiety of patients,poor evaluation of self-body,poor social support will reduce their quality of life,so helping patients reshape their self-confidence including sexual attraction,and appropriate psychological intervention is also necessary.We can use the methods including popularizing knowledge of breast cancer to family members of patients,friends,and classmates,helping them participate in rehabilitation plans,and instructing their spouses to provide patients with care and support,All above can improve the quality of life of breast cancer patients.In summary,in addition to the treatment of the disease,breast surgeons need to pay close attention to the patient's psychological status,master the necessary psychological counseling measures,and do timely psychological intervention to improve the quality of life of breast cancer patients.
