Objective: To investigate the clinical value of shear wave wave elastography(SWE)in differentiating liver tumors and the relationship between the size of the tumor,the degree of tumor differentiation and the mean value of tumor elastic modulus(Emean).Methods: 57 patients with hepatic hemangioma and 63 patients with liver cancer were selected.The mean elasticity of each lesion and the surrounding normal tissues was measured and recorded by SWE technique and calculate the elastic ratio(Eratio).The Emean and Eratio of each type of liver tumor is expressed as mean±standard deviateon.The Mann-Whitney test was used to compare the Emean and Eratio of hepatic hemangioma and liver cancer.The cut-off values and the area under the curve(AUC)of hepatic hemangioma and liver cancer were calcualated with ROC curve.The spearman grade correlation was used to investigate whether there was a correlation between the size of the tumor.The Emean of the three groups of differentiation degree of HCC were compared by ANOVA.Results: The Emean of hepatic hemangioma was 19.38 ±8.69 kPa,and that of liver cancer was 39.08 ±18.03 kPa.The difference was statistically significant(P < 0.05).The critical value of Emean for the diagnosis of hepatic hemangioma and liver cancer was 24.65 kPa,the sensitivity was 78.6 % and the specificity was 90.0%.The area under ROC curve was 0.875;The critical value of Eratio for the diagnosis of hepatic hemangioma and liver cancer was 3.53,the sensitivity was 74.2%,and the specificity was 74.1%.The area under the ROC curve was 0.798.There was a positive correlateion between Emean of hepatic hemangioma and the size of the lesions(P < 0.05),and there was a positive correlation between the Emean of liver cancer and the size of the lesions.There was a statistically significant between the differentiation degree of HCC and the Emean(P < 0.05).Conclusion: SWE can be used to quantitatively evaluate the hardness information of liver tumors by obtaining the Emean and the Eratio.It has a broad prospect of clinical application in distinguishing the nature of liver cancer and hepatic hemangioma.
