Abstract/Summary:
Objective:Comparative analysis of the diagnostic efficacy of conventional ultrasound,contrast-enhanced ultrasound(CEUS)and shear wave elastography(SWE)in the diagnosis of benign and malignant liver tumors.To explore the clinical application value of CEUS combined with SWE in the diagnosis of liver tumors.Methods:Routine ultrasound,CEUS and SWE examinations were performed on 135 liver tumor patients who were treated in our hospital from September 2019 to December 2020.Use pathological examination(surgical resection/puncture biopsy)or final clinical diagnosis results as the gold standard to analyze the features of conventional ultrasound,CEUS and SWE of the lesion,and calculate the sensitivity,specificity and accuracy of conventional ultrasound,CEUS,SWE and CEUS+SWE combined diagnosis of liver tumors.The chi-square test was used to compare whether the difference in diagnostic accuracy of the four inspection methods was statistically significant,and P<0.05 indicated that the difference was statistically significant.Results:1.Among the 156 liver tumor lesions in 135 patients,65 were benign lesions:40hepatic hemangioma,16 regenerative nodules in liver cirrhosis,9 focal nodular hyperplasia of liver;91 malignant lesions:48 hepatocellular carcinoma,31 metastatic liver cancer,12 intrahepatic cholangiocarcinoma.2.The three inspection methods have different sensitivity,specificity and accuracy for the qualitative diagnosis of liver tumors.Conventional ultrasound was67.0%,60.0%,and 64.1%,CEUS were 87.9%,89.2%,and 88.5%,SWE were 80.2%,75.4%,and 78.2%,respectively.Both CEUS and SWE are significantly better than conventional ultrasound in diagnosing liver tumors,and the difference is statistically significant(χ~2=7.558、P=0.006,χ~2=25.581、P=0.000).3.SWE was slightly less accurate than CEUS in diagnosing liver tumors.The sensitivity,specificity,and accuracy of the CEUS+SWE were 95.6%,93.8%,and94.9%,respectively,which were significantly better than SWE and higher than CEUS,and the difference was statistically significant(χ~2=18.599、P=0.000,χ~2=4.196、P=0.041).The comparison of the diagnostic accuracy of the four different inspection methods showed CEUS+SWE>CEUS>SWE>conventional ultrasound results,and the differences were statistically significant(P<0.05).Conclusion:Conventional ultrasound has a high detection rate of liver tumors and a general differential diagnosis ability.Both CEUS and SWE have high diagnostic accuracy.The two can complement each other’s diagnostic information and complement each other’s advantages.The CEUS+SWE diagnostic mode significantly improves the overall diagnostic efficiency of the early qualitative diagnosis of liver tumors in the single examination mode of CEUS and SWE,and has certain clinical application value.
