Objective To explore the differences in personal history,current medical history,co-morbidity and laboratory examination between EP,GPP and PV patients,summarize the characteristics and further explore the possible risk factors of different types of psoriasis in Tianjin.Methods The clinical data of EP,GPP and PV patients including general information,personal history,co-morbidity,laboratory examination comes from the dermatology ward of our hospital from June 1,2016 to June 30,2018,then established the information database by Excel 2003 and processed by SPSS.21.0 software.Results1.The proportion of women in GPP group was significantly higher than that in PV group(P2<0.001);the proportion of patients in EP group whose psoriasis course was longer than 15 years was significantly higher than that in PV group(p1=0.001).The positive proportion of family history in GPP group was significantly lower than that in PV group(P2=0.038).The average hospitalization times of EP and GPP group were higher than those of PV group,and the repeated hospitalization rate was significantly higher than that of PV group(p1=0.017,P2=0.048).2.The predisposing factors of the three groups were irregular withdrawal,unreasonable oral or external medication and infection,but there was no significant difference between the three groups.3.The proportion of patients in EP and GPP group who used acitretin in the past was higher than that in PV group(p1=0.019,P2=0.025).4.The main symptoms of the three groups were pruritus,the proportion of pain in EP and GPP group was significantly higher than that in PV group(p1<0.001,P2<0.001).The most common concomitant symptoms in EP and GPP groups were fever and insomnia.Other concomitant symptoms including fatigue,loss of appetite,joint pain,which proportion was higher than that in PV group.5.EP,GPP and PV patients all have high co-morbidity of systemic diseases.The most common co-morbidities are overweight/obesity and hyperlipidemia,followed by hypertension,fatty liver, hyperuricemia,type 2 diabetes mellitus and cardiovascular diseases.6.In laboratory examination,anemia rate in EP group was significantly higher than that in PV group(p1<0.001);hypokalemia and hypophosphatemia rate in GPP group was significantly higher than that in PV group(p2<0.001);WBC count,NEU%,SAA,CRP,ESR,TP and ALB in EP and GPP group were significantly higher than those in PV group(p<0.001).There was no significant difference in the proportion of abnormal liver enzymes and hypocalcemia among the three groups.7.Logistic regression analysis showed that psoriasis duration more than 15 years(OR=2.422,95% CI 1.424-4.119),hospitalization times more than 2 times(OR=2.132,95% CI 1.078-4.216)were risk factors for EP.Women(OR=3.103,95% CI 1.753-5.491),hospitalization times more than 2 times(OR=2.055,95% CI 1.023-4.131)were risk factors for GPP.Conclusions1.The course of psoriasis,average hospitalization times and repeated hospitalization rate of severe psoriasis patients were higher than PV patients.There was no significant difference in the onset age of psoriasis,smoking and drinking history.2.Infection,irregular withdrawal and unreasonable use of drugs are common causes of severe psoriasis.3.The proportion of severe psoriasis patients treated with acitretin in the past was higher than that of PV group.4.The conscious symptoms and accompanying symptoms of severe psoriasis are heavier than those of psoriasis vulgaris.5.Both severe psoriasis and psoriasis vulgaris have high incidence of systemic diseases,but there was no clear correlation between the types of psoriasis and the complications.6.In severe psoriasis patients,the elevation level of infectious indicators and depression level of nutritional indicators was higher than that of psoriasis vulgaris.7.The main risk factors of EP were psoriasis duration and hospitalization times;the main risk factors of GPP were women and hospitalization times.
