Clinical Study Of Yinshandan Decoction On The Mechanism Of Damp-heat Accumulation In Acute Gouty Arthritis

Objective: To observe the mechanism of treatment of acute gouty arthritis(AGA)by Yinshandan decoction and discuss its effect.Method : Sixty patients with Damp-heat accumulation acute gouty arthritis were selected and randomly divided into treatment group(30cases)and control group(30 cases).The control group was treated with non-steroidal anti-inflammatory drugs(Etoricoxib Tablets),while the treatment group was treated with Yinshandan decoction combined with non-steroidal anti-inflammatory drugs(Etoricoxib Tablets),one course of treatment a week and two courses of treatment were observed.The degree of pain,fever,CRP,ESR,UA,T lymphocyte,liver and kidney function and other indicators,through statistical analysis,the results were obtained,and combined with the academic thinking of the tutor and the composition of the prescription,the mechanism was analyzed,and finally the research conclusions were drawn.Result:1.According to the statistics of the curative effect of TCM symptoms,the clinical cure rate of the treatment group was 13.3%,the marked effective rate was 43.3%,the effective rate was 36.7%,the ineffective rate was 6.7%,and the total effective rate was 93.3%;the clinical cure rate of the control group was 3.3%,the marked effective rate was 30.0%,the effective rate was 46.7%,the ineffective rate was 20.0%,and the total effective rate was 80.0%.2.Comparison of TCM syndrome integrals: There was significant difference in TCM syndrome integrals between the two groups before and after treatment(P < 0.01);there was significant difference in TCM syndrome integrals between the two groups after treatment(P <0.05).3.Comparison of VAS scores:There was significant difference in VAS score between the two groups before and after treatment(P <0.01),and there was significant difference in VAS score between the two groups after treatment(P < 0.05).4.Comparison of laboratory indicators: CRP and ESR indicators of the two groups decreased before and after treatment,and there were significant differences(P <0.01).There were significant differences in CRP and ESR indicators between the two groups after treatment(P <0.05).There was significant difference in UA level before and after treatment in the treatment group(P < 0.01),but there was no significant difference in UA level before and after treatment in the control group(P > 0.05).There was significant difference in UA level between the two groups after treatment(P < 0.05).The levels of CD3+,CD4+,CD8+,CD4+/CD8+ in the two groups were significantly different before and after treatment(P < 0.01),and the levels of CD3+,CD4+,CD8+,CD4+/CD8+ in the two groups were significantly different after treatment(P < 0.05).5.Comparison of safety: There was only one case of mild gastrointestinal discomfort in the treatment group.These were mild adverse reactions.Symptoms improved after symptomatic treatment.Noblood system,vital signs,liver and kidney damage were observed.Reaction;2 cases of gastrointestinal discomfort occurred in the control group,and no other adverse reactions were found.Conclusion:1.Yinshandan decoction has remarkable curative effect in treating acute gouty arthritis of damp-heat accumulation type;2.Yinshandan decoction can effectively reduce symptoms such as joint pain index,swelling index and joint function,reduce the levels of inflammatory indicators such as ESR and CRP,indicating its anti-inflammatory and analgesic effects;3.Yinshandan decoction can reduce UA indicators of patients,indicating that Yinshandan decoction can reduce uric acid;4.Yinshandan decoction can reduce uric acid.It can regulate T lymphocyte subsets,indicating that Yinshandan decoction can regulate immune function;5.Yinshandan decoction can significantly improve the symptoms of damp-heat accumulation acute gouty arthritis,reduce inflammatory activity,regulate immunity and other laboratory indicators,through anti-inflammatory analgesia,reduce uric acid,regulate immunity and other acute gouty arthritis,and no obvious toxic side effects,it is worth in clinical practice.Further promotion and application.
