Objects:To study the effects of individualized fortification of breast milk on growth,nutrition and safety in premature infants in China during hospital stays.Methods:A prospective controlled study was conducted in shanghai children's medical center during August 1,2015 to January 31,2016.Premature infants were divided into two groups: individualized fortification of breast milk group(experimental group:those fed by breast milk that was provided by their own mother over 50 percent during hospital stays.preterm formula was used for those who didn't have enough human milk),and preterm formula group as control group(those fed fully by preterm formula).The head circumference,height,weight growth and enteral nutritional intakes and the rate of feeding intolerance,NEC,sepsis were compared between the two groups.Results:Among the 46 cases included,19 were in the experimental group,and 27 were in the control group.There was no difference in gestational age and birth weight between the two groups(31.95± 1.77 w vs32.14 ± 1.51 w,p=0.698;1696 ± 243 g vs 1665 ±208g,p=0.645).There was no difference in the length of hospitalization time and regaining birth weight between the two groups(31.21 ± 11.03 d vs 29.22 ± 10.77,p=0.545;11.11 ± 3.54 d vs 12.56 ± 4.01 d,p=0.212).There was no difference in weight,length,head circumference gain between the two groups(14.38 ± 5.51g/kg·d vs15.83 ± 8.71g/kg·d,p=0.528;0.99 ± 0.52m/w vs 0.84 ± 0.73cm/w,p=0.449;0.66 ±0.36cm/w vs 0.63±0.42cm/w,p=0.813.There was no difference in pre-hospital infection rate and NRDS between the two groups(0.74 vs 0.85,p=0.456;0.21 vs 0.33,p=0.510).There was no difference in incidence of feeding intolerance and sepsis between the two groups(0.16 vs 0.30,p=0.320;0 vs 0.04,p=1.00).NEC rate of the experimental group is less than the control group(0.05 vs 0.15,p=0.03).There was no difference in the enteral energy intakes at every weekend(7th day,14 th day,21 th day...).There was no difference in the days for achieving total energy intake of 100kcal/(kg·d),110 kcal/(kg·d)and 120ml/(kg·d),150ml/(kg·d)through enteral feeding between the two groups either(23.42±11.09 d vs22.29±12.05 d,p=0.749;26.16±11.20d vs25.44±13.83 d,p=0.853;20.68±9.27 d vs21.87±12.26 d,p=0.726;24.89±11.37 d vs26.52±13.78 d,p=0.675).There was no difference in biochemical parameters between the two groups.Conclusions:There was no difference in the length of hospitalization time and regaining birth weight,weight,length,head circumference gain and enteral energy intakes between the two groups.The weight gain in experimental group could nearly reach the intrauterine growth target.There was no difference in incidence of feeding intolerance and sepsis between the two groups.NEC rate of the experimental group is less than the control group.
