Objective:To explore the best nutrition plan suitable for the growth and development of very low birth weight infants by analyzing nutrition plan,weight gain,complications and neurological assessment.Content and methods:We choose infants diagnosed with very low birth weight infants(VLBW)as the research object,who accepting treatment during January 2014 to December 2017 in Guangdong Women and Children Hospital,which affiliated to Guangzhou Medical University.Children with digestive tract abnormalities,congenital genetic metabolic diseases,midlife abandonment or death were excluded and finally included in 649 infants.We divide to fortification group and non-fortified group according to adding fortifier.Fortification group divide by dose to half fortification group and the full fortification group,divide by adding time to early fortification group and late fortification group.Through the retrospective analysis of the growth and development of very low birth weight infants,researching the effects of human milk fortification.SPSS16.0 software was used for data processing and analysis.Results:The research observed 649 infants,306 fortification group and 349 non-fortified group,two groups general situations do not have statistical significance.Fortification group grow better than non-fortified group,?8.74g/(kg.d)vs 8.28g/(kg.d),P=0.028?.By the time Fortification group has lower EUGR rate(85.5%vs91.7%,P=0.015).There is no statistical significance between the fortification group and the non-fortified group in complications.The abnormal rate of EEG in the fortification group is lower(39.9%vs40.8%,P=0.003),the abnormal rate of MRI in the fortification group is also lower(23.9%vs24.8%,P=0.001).Full fortification group 106 cases,half fortification group 200 cases.The statistical difference between two groups in general situations ? complications ? NBNA assessment?MRI is same(P>0.05).Full fortification group grow better than half fortification group?8.99g/(kg.d)vs8.32g/(kg.d),P=0.026?.The abnormal rate of EEG in the full fortification group is higher than the half fortification group(53.8%vs32.5%,P=0.032).Early fortification group 30 cases,late fortification group 276 cases.The statistical difference between two groups in general situations?complications?NBNA assessment is same(P>0.05).The abnormal rate of EEG in the early fortification group is higher than the late fortification group(50.0%vs38.8%,P=0.001),the abnormal rate of MRI in the early fortification group is lower than the late fortification group(23.3%vs23.9%,P=0.000).Conclusion:1.Human milk fortification can increase the rate of weight growth of very low birth weight infants,reduce the occurrence of EUGR.2.Full fortification is more beneficial to the weight gain.3.There was no significant difference in the effects of different fortification methods and the timing of fortification on the complications.4.The abnormal rate of EEG and MRI is lower in the fortification group,which can reflect fortification feeding lead to better neurological prognosis.
