A Survey Of Status Of The Maintenance Of Peripherally Inserted Central Catheter In Tertiary Hospitals In Shanghai

ObjectiveThe purpose of this study was to to describe the practices involved in the maintenance of peripherally inserted central catheters(PICC)in Grade A tertiary hospitals in Shanghai,compare the findings with standards ‘Nursing practice standards for intravenous therapy'.MethodsThe combination of questionnaire investigation and semi-structured interview was used in this study.Researchers design a questionnaire on basis of ‘Nursing practice standards for intravenous therapy'and ‘Infusion Nursing Standards of Practice',to investigate on status of the maintenance of peripherally inserted central catheter in Grade A Tertiary Hospitals in Shanghai,semi-structured interviews are conducted on 8 PICC professional nurses to explain and supplement the quantitative results.Results 1.There were 30(81.1%)hospitals applied PICC technology,applied PICC(9.73±3.85)years,27(90.0%)hospitals set up PICC Clinics.2.The department with a larger volume of PICC catheter include,24 oncology departments,22 general surgery departments,18 hematology departments,11 neonatal departments,a total of 75 departments.In the survey of 75 departments,96.1% departments use transparent dressing at the puncture site,61.3% departments disinfected the skin with 2% chlorhexidine alcohol,61.3 % departments use Stalock to fixthe catheter,90.7% departments used heparin cap,76.0% departments used positive pressure connectors,74.7% lock PICC with saline solution,all departments used a 10 ml syringe to flush PICC,45.3% departments used special care package,PICC maintenance fees for 5~163 yuan,P25=27yuan,P50=30yuan,P75=45yuan.3.There was no strict education,professional title requirements for the PICC maintenance nurses in more than 70% hospitals,PICC maintenance training institutions includ the nursing institute,hospital,PICC catheter manufacturers.There were a variety of patterns of PICC maintenance personnel arrangements.4.The contents of three aspects were summarized from the interview data as a supplement to the quantitative results,PICC maintenance practices,PICC maintenance personnel qualifications,training and arrangement model,the development trend of PICC maintenance.Conclusions 1.PICC technology was widely used in Shanghai,PICC Clinics coverage rate was high,but it developed inbalancely.2.The practices involved in the maintenance of PICC in Grade A tertiary hospitals in Shanghai were close to the standard,but there were still differences between the hospitals.The utilization rate of the chlorhexidine alcohol and the special care package was low,there were many types of the dressing,the connector and the ways of the catheter fixation,charge differences.3.PICC maintenance personnel qualification requirements exist diversity.4.PICC maintenance personnel training resources were limited.
