Background: Along with the improvement of medical care in China,national population is aging,therefore the account of geriatric surgery is also largely boosting.As a unique part of the population,geriatric patients are often associated with altered physical characteristics,and lower tolerance to a traumatic attack such as surgery.They are often associated with higher incidence of postoperative complications.Among all the postsurgical complications,postoperative pulmonary complication(PPCs)is a main contributor to patients' morbidity and mortality.Therefore,investigations on reducing PPCs is of vital urgency and importance to modern medicine.As the two most popular airway management methods,laryngeal mask airway(LMA)and endotracheal intubation(ETI)both have their unique advantages in specific clinical settings.They are compared under various clinical conditions.However,publications that compare their influence on PPCs is scarce.For the purpose of helping clinicians making better decisions on airway management during geriatric surgery,this study contains a single-center retrospective study and a multi-center clinical trial to compare the impact of LMA and ETI on PPCs.Method:1.In the first part of the study,we collected clinical data from over 2000 patients,after making sure they meet the inclusion and exclusion criteria.According to their method of mechanical ventilation,we divided them into 2 groups,LMA and ETI group.After that,we compared their demographic data,and other clinical measurements such as incidence and types of PPCs.2.Based on the finding from part 1,we furtherly designed a more comprehensive clinical trial.After recruiting 6000 participants from 17 clinical center,we will compare the incidence,mortality and mortality of PPCs and evaluate the influence of ventilation method on the development of PPCs.Result:1.We learned from this retrospective study that,in comparison with ETI group,LMA group is associated with lower peak WBC count,lower incidence of atelectasis and pulmonary embolism.Demographic data,incidence of ICU administration,duration of hospitalization,mortality rate is comparable between the two groups.2.After organizing the ~500 cases that we have at hand,we compared demographic data,incidence of PPCs,mortality rate at different time points,incidence and duration of ICU admission,total hospitalization duration,ect between the two groups.And the current statistical analysis all showed insignificant differences between the two groups.ConclusionDespite the conclusion from part1: in comparison with ETT,LMA is associated with lower incidence of atelectasis and pulmonary embolism,based on the analysis of part2,the influence of LMA and ETT on PPCs is comparable.
