Analysis Of Diabetic Foot Ulcer's Prognosis And Its Association With Potential Factors

Objective To investigate the mortality,nonfatal cardiovascular events' occurennce,ulcer's healing or recurrence rate of patients hospitalized in Shanghai Yuanyang Dibetic Foot Management Center for first occurrence of DFU through regular follow-up;and to determine the impact of corrected QT interval and serum C peptide level on endpoints of this cohort.Methods The clinical materials of 732 DFU patients were collected who were hospitalized by Shanghai Yuanyang Dibetic Foot Management Center for the first time.These patients were regularly followed up until June 2015 or until their death.Ulcers' healing or recurrence,patients' deaths and their causes,nonfatal cardiovascular events and corresponding dates were recorded.COX analysis was used to make Kaplan-Miere survival curves and to determine the factors related with outcomes of DFU.Assort DFU patients' clinical materials including QTc as well as serum C peptide level,so as to investigate the associations of DFU disease severity or outcomes with QTc or C peptides.Results Ulcers' healing rate in 1,3,5 years was 67.23%?69.46%,74.26%;recurrence rate was 19.34%?38.9% and 48.9%;all-cause mortality was 10.23%?21.05% and 34.26%;cardiovascular mortality was 5.88%?11.29% and 18.44%;NCCVE rate was 9.89%?18.79% and 28.69%;major amputation rate was 2.1%?2.9% and 4.3%.QTc prolongation was correlated with allcause mortality,cardiac mortality and cardiovascular mortality;and it was also the independent risk factor to cardiac mortality.Serum C peptide level of different time points were all relatively lower in DFU patients than normal;and have no correlation with Wagner or infection degrees(P>0.05).Fasting C peptide level was an independent protecting factor of ulcers' healing.Conclusion DFU patients' conditions are complicated,have a higher mortality and cardiovascular occurrence rate.QTc prolongation and serum fasting C peptide could be used as good referential factors.
