Part1 SPECT Quantitative Analysis of coronary physiology:Comparison with PETObjective:To evaluate the diagnostic value of quantitative rest and stress 99mTc-MIBI SPECT dynamic myocardial perfusion imaging?DMPI?on Saanen:Comparison with PET.To clarify the accuracy and feasibility of 99mTc-MIBI SPECT DMPI quantitative analysis.Method:A total of Five Saanen?25-30Kg?whose rest and ATP stress DMPl were acquired using13N-NH3 PET and 99mTc-MIBI SPECT and quantitative analysis was performed using the homemade MBF program and the MyoFlowQ?Taiwan?program to obtain resting myocardial blood flow,stress myocardial blood flow,and Myocardial flow reserve.The results obtained by SPECT and PET were compared and correlated.Results 1.Of the 8 experimental sheep,3 died of anesthesia,1 case showed poor results,and 4 cases were included in the analysis.2.The Global RMBF,SMBF and MFR quantified by 99mTc-MIBI SPECT were 1.59±0.38,2.34±0.89,1.41±0.29,respectively,and the Global RMBF,SMBF and MFR quantified by13N-NH3 PET were 1.44±0.47,2.32±1.01,1.59±0.37,respectively.There was no significant difference in RMBF,SMBF and MFR between the two groups?P>0.05?.In addition,in the LAD,LCX and RCA of Saanen,the RMBF,SMBF and MFR quantified by SPECT and PET showed no significant difference?P>0.05?.3.99mTc-MIBI SPECT DMPI has a strong linear correlation with 13N-NH3 PET MPI on RMBF and SMBF of Global,LAD,LCX and RCA?RMBF:r=0.885,P<0.001;SMBF:r=0.871,P<0.001?,while the MFR of both has a moderate linear correlation?MFR:r=0.653,P<0.05?.Conclusions:Compared with the"gold standard"13N-NH3 PET DMPI,its quantitative analysis index of 99mTc-MIBI SPECT DMPI is similar.Therefore,99mTc-MIBI SPECT quantitative analysis can be used for the assessment of myocardial perfusion.Part2 Quantitative Analysis of SPECT Myocardial Perfusion Imaging in Application of Coronary Microvascular DysfunctionObjective: To observe the changes of quantitative and semi-quantitative indexes before and after modeling in the coronary microvascular dysfunction model of Saanen by rest and stress99mTc-MIBI SPECT.To clarify the application value of SPECT MPI quantitative analysis methods in CMVD.Method: A total of Seven Saanen?25-30Kg?were established CMVD animal model by microsphere embolization.Before and after the modeling,rest and stress99mTc-MIBI SPECT dynamic MPI was performed,and static acquisition was performed 1 hour after each dynamic acquisition.The results were analyzed by the built-in software for qualitative and semi-quantitative analysis.Qualitative analysis includes the location of ischemia and the degree of ischemia,and compared the ischemic site after modeling with pathology.Semi-quantitative analysis was performed using the five-point method: 0 was normal,1 was normal,2 was mild myocardial ischemia,and 3 was moderate myocardial ischemia,4 is divided into severe myocardial ischemia.Calculating the total score of the left ventricular myocardium in the myocardial imaging of the load myocardial imaging,ie summed stress score?SSS?: normal: 0-3 points;mild ischemia:4-8 points; moderate ischemia:9-13 points;severe ischemia: >13 points.Quantitative analysis was performed by the Myo Flow Q?Taiwan?program.The quantitative values of PET after modeling were compared with those before modeling.Results 1.Of the 7 Saanen,3 died and 4 underwent complete myocardial perfusion imaging before and after modeling.2.A total of 68 left ventricular myocardial segments were evaluated in 4 CMVD models.The fractions with reduced nuclide distribution in SPECT MPI were 6.Compared with pathology,the ischemic position roughly matches.In the abnormal segment of SPECT MPI,3 were partial reversible defects,2 were fixed defects,and 1 was reverse redistribution.3.The analysis was performed by semiquantitative index.Of the 4 CMVD models,2 were normal and 2 were mildly ischemic.4.The RMBF of Global and three vessels after modeling by SPECT MPI was not significantly different from that before modeling?P>0.05?,the SMBF of Global,LAD,LCX and RCA were significantly lower than those before modeling?P<0.05?,and the MFRof Global,LAD,LCX and RCA were also significantly lower than those before modeling?P<0.05?.Conclusions: 99mTc-MIBI SPECT MPI quantitative analysis can assess the perfusion of CMVD.In addition,quantitative analysis of SPECT MPI is superior to qualitative and semi-quantitative analysis in the diagnosis of CMVD.
