Objectives This study was to observe and compare the clinical efficacy of Shangke Hongyao spray combined with sodium aescinate and sodium aescinate alone in the treatment of swelling and pain before ankle fracture surgery,to evaluate the clinical effect of Shangke Hongyao Spray,in order to provide more effective treatment for swelling and pain of ankle fracture before operation.Methods From January 2018 to December 2018,60 patients with ankle fracture admitted to the Second Department of Traumatic Orthopedics of Cangzhou Integrated Hospital of Traditional Chinese and Western Medicine(hereinafter referred to as our hospital)were randomly divided into two groups: the group of Shangke Hongyao Spray plus Sodium aescinate(hereinafter referred to as the treatment group)and the group of Sodium aescinate(hereinafter referred to as the control group),30 cases each.Both groups were treated with the same routine treatment.The control group was treated with sodium aescinate(Chinese medicine standard word H20003240,Shandong Green Leaf Pharmaceutical Co.,Ltd.,20mg)on the second day of admission.Sodium aescinate of20 mg was dissolved in 0.9% N.S250 ml or 5% G.S250 ml,intravenously dripped for 1/day until the day of operation.The treatment group was treated with the self-made Shangke Hongyao Spray and Sodium aescinate on the second day of admission.Shangke Hongyao Spray was evenly sprayed on the affected area at 08:00,14:00 and 20:00 a day,with the dosage of 10 ml each time.The same drug and method of Sodium aescinate were used until the day of operation.The swelling value and pain VAS scores of the two groups were evaluated on the day of admission and after 3 or 5 days of treatment respectively;the efficacy of swelling relief after 3 or 5 days of treatment was evaluated;the swelling score of the affected area was evaluated daily after admission,and the waiting time before operation was calculated;the color Doppler ultrasound of the lower extremity arteries and veins was examined one day before operation to record the occurrence of deep venous thrombosis(DVT)of the lower extremities.After 5 days of treatment,liver and kidney functions were tested and adverse drug reactions were evaluated.All data were processed and analyzed by statistical software SPSS17.0 to evaluate the clinical efficacy of two treatment methods for swelling and pain before ankle fracture.Results 1 In this study,60 patients were eventually collected and randomly divided into treatment group(30 cases)and control group(30 cases).There was no significant difference in sex ratio,age,injury side,etiology,healthy side circumference,LanguesHansen classification of fracture and swelling degree between the two groups(P>0.05).In this study,no drug-related adverse reactions occurred in both groups.2 Comparisons of swelling value: There was no significant difference between the two groups before treatment(P>0.05),and there was comparability.Comparisons between the two groups:after 3 days of treatment,although the swelling value of the treatment group was smaller than that of the control group,there was no significant difference(P>0.05).After 5 days of treatment,the swelling value of the treatment group was significantly smaller than that of the control group,with statistical significance(P<0.05).Intra-group comparison: The swelling value of patients in the treatment group before treatment,3 days and 5 days after treatment were compared,the difference was statistically significant(P<0.05).The swelling value of the control group was compared before treatment,3 days and 5 days after treatment,the results showed that the swelling value decreased after 3 days of treatment,but there was no significant difference(P>0.05).The swelling value decreasedsignificantly after 5 days of treatment compared with that before treatment,with statistical significance(P<0.05).3 Evaluation and comparison of curative effect: There was a significant difference in the detumescence rate between the two groups after 3 days of treatment(P<0.05);there was a significant difference in the detumescence effect between the two groups after 5 days of treatment(P<0.05).The excellent rate of the treatment group was 56.67%,while that of the control group was 6.67%.4 VAS score comparison:There was no significant difference in VAS score before treatment between the two groups(P>0.05),which was comparable.The VAS score of pain in the treatment group was significantly lower than that in the control group after 3 or 5 days of treatment(P<0.05).Intra-group comparison: The treatment group before treatment,3 days after treatment,5 days after treatment,the difference was statistically significant(P<0.05),while the control group before treatment,3 days after treatment,5 days after treatment,the difference was statistically significant(P<0.05).5 Preoperative waiting time: There was significant difference between the two groups(P<0.05).6 Comparing the incidence of DVT: There was no significant difference in the incidence of DVT between the two groups during treatment(P>0.05).7 Comparisons of TCM syndromes: There was significant difference between the two groups after 3 days of treatment(P<0.05);there was significant difference between the two groups after 5 days of treatment(P<0.05).The excellent rate of the treatment group was 96.67%,and that of the control group was 73.33%.Conclusions 1 The Shangke Hongyao spray,sodium aescinate combined group and sodium aescinate single group can eliminate the swelling and reduce the degree of pain in patients with ankle fracture before operation to a certain extent.2 Shangke Hongyao spray and sodium aescinate combined group can significantly reduce the swelling and pain of patients with ankle fracture before operation,which is better than sodium aescinate group,and can significantly shorten the waiting time before operation.3 Shangke Hongyao Spray combined with sodium aescinate can improve swelling and pain of ankle joint fracture before operation.It is a more effective clinical scheme for treating swelling and pain of ankle joint fracture before operation.It is worthy of further popularization and application.Figure 3;Table 15;Reference 127.
