ObjectivesTo investigate the value of Caprini risk assessment model in predicting venous thromboembolism(VTE)in patients undergoing gynecological surgery and the independent risk factors of venous thromboembolism in gynecological patients during anesthesia,in order to establish a anaesthetic period for gynecological diseases in China.The VTE risk assessment model provides a reference.MethodsA retrospective analysis of the clinical cases of 153 patients diagnosed with VTE after gynecological department surgery in the Department of Gynecology,North China University of Technology from January 2015 to January 2018,was set as observation group,and according to 1 The principle of 2 was randomly selected from the306 patients with non-VTE in the same department at the same time as the control group by random number method.The relationship between 18 risk factors in the Caprini model and other clinical factors during anesthesia and venous thromboembolism after gynecological surgery were compared by t-test or chi-square test.Then binary logistic regression analysis was performed.To understand the independent risk factors of VTE in patients after gynecological surgery,the Caprini model was used to score and risk grades,and the relationship between risk grade and risk of concurrent VTE was analyzed.ResultsIn the Caprini model,severe lung disease,abnormal lung function,malignant tumor,leg swelling,pregnancy or 1 month after delivery,oral contraceptive or estrogen and progesterone replacement therapy,establishment of central venous access,large open surgery,VTE family history,VTE disease history The 10 categories are independent risk factors for VTE after gynecological diseases.Among the factors most closely related to the occurrence of VTE is the family history of VTE(OR=111.580),followed by oral contraceptives or estrogen and progesterone replacement therapy(OR=24.596);and other anesthesia methods during perioperative anesthesia The five types of indicators,surgical position,application of inhaled anesthetics,application of hemostatic drugs,and D-dimer classification are independent risk factors for VTE in patients undergoing gynecological diseases.The most closely related factor to VTE is anesthesia(OR=896.004),followed by D-dimer fractionation(OR=33.032).The average score of CapriniRAM in the observation group was significantly higher than that in the control group[(7.12±1.61)points vs.(3.90±2.19)points],and the difference was statistically significant(P<0.001).The higher the risk rating assessed by CapriniRAM,the greater the number of patients with VTE identified,and the difference was statistically significant(?~2=190.217,P<0.001).The risk of VTE in very high-risk groups was 138.112 times higher than that of non-very high-risk groups(95%CI:49.300~386.913).Conclusions1 Caprini RAM can stratify VTE risk in patients undergoing gynecological surgery.2 For people with higher Caprini score,the chance of thrombotic events is increased.3 CapriniRAM can effectively identify the risk degree of thrombotic events in gynecologic patients.Figure2;Table12;Reference 277...
