ObjectivesInvestigate the role of serum IGF-1 in the pathogenesis of diabetes mellitus combined with thyroid disease by analyzing the difference of serum IGF-1 in patients with diabetes mellitus and different thyroid diseases.To provide theoretical basis for the prevention and treatment of thyroid diseases in diabetes patients.Methods200 patients from August 2017 to May 2018 who suffered from diabetes mellitus and were admitted to the Department of Endocrinology,Affiliated Hospital of North China University of Technology were chosen,and according to whether they were combined with thyroid disease or not,were divided into diabetes mellitus combined with thyroid disease groups?200 cases?as case group and simple diabetes mellitus group?50cases?as control group.The case group included diabetes mellitus with hyperthyrea,diabetes mellitus with hyperthyroidism and diabetes mellitus with thyroid nodule,then compared with the control group.Sex,age,BMI,duration of diabetes,fasting blood glucose?FPG?,2 h postprandial blood glucose?PG?,glycosylated hemoglobin?HbA1c?,fasting insulin?FINS?,2 hinsulin?INS?,fasting C peptide,2 h postprandial C peptide,triacylglycerol?TG?,totalcholesterol?TC?,high density lipoprotein?HDL?,low density lipoprotein?LDL?and insulin resistance index?IR?,the serumIGF-1,thyroid peroxidase antibody?TPOAb?,and thyroglobulin antibody?TgAb?of the two groups were determined and compared.Excel was used to establish the database and all the data were input into SPSS 21.0 software for statistical analysis.The measured data in accordance with normal distribution were expressed as?x?s?,and the t-test was used in the comparison between the two groups.The non-normal distribution data were expressed by M?P25,P75?,and the two groups were compared by the summation test.The?2 test is used in the analysis of counting data.The influencing factors of diabetes mellitus complicated with thyroid disease were analyzed by unconditional two-class logistic regression analysis.Results 1 The diabetes mellituscomplicated with thyroid disease group suffered from age,duration of diabetes,FPG,IR,FINS,HbA1c,TC,HDL,TgAb were higher than the simple diabetes mellitusgroup,and LDL was lower than the simple diabetes mellitus group.The serum IGF-1 level in the diabetes mellitus with thyroid disease group was lower than that in the simple diabetes mellitusgroup and when the serum IGF-1?157ug/L,the risk of diabetes mellitus combined with thyroid disease was increased by 1.518 times.Taking the serum IGF-1,age,duration of diabetes,FPG,IR,FINS,HbA1c,TC,HDL,TgAb and LDL which were statistically significant indicators as independent variables,and whether thyroid disease as the dependent variable,the logistic regression of unconditional two-class classification was performed showed that the duration of diabetes,IR and the serum IGF-1were the influencing factors of diabetes mellitus complicated with thyroid disease.2 The diabetes mellitus complicated with the hyperthyrea group suffered from duration of diabetes,FPG,IR,FINS,TgAb and TPOAb were higher than the simple diabetes mellitus group,and TG and TC were lower than the simple diabetes mellitusgroup.The serum IGF-1 level in the hyperthyrea group was lower than that in the simple diabetes mellitus group and when the serum IGF-1?151ug/L,the risk of diabetes mellitus combined with hyperthyrea was increased by 3.958 times.Taking the serumIGF-1,duration of diabetes,FINS,FPG,IR,TPOAb,TgAb,TG and TC which were statistically significant indicators as independent variables,and whetherhyperthyrea as the dependent variable,the logistic regression of unconditional two-class classification was performed showed that FINS,IR and the serum IGF-1 were the influencing factors of diabetes mellitus complicated with hyperthyrea.3 The diabetes mellitus complicated with the hypothyroidism group suffered from duration of diabetes,BMI,FPG,IR,HbA1c,HDL,TG,TC and TgAb were higher than the simple diabetes mellitus group,and LDL were lower than the simple diabetes mellitus group.The serum IGF-1 level in the hypothyroidism group was lower than that in the diabetes mellitus diabetes group and when the serum IGF-1?166ug/L,the risk of diabetes mellitus combined with hypothyroidism was increased by 2.222 times.Taking the serum IGF-1,duration of diabetes,BMI,HbA1c,FPG,TG,TC,HDL,LDL,TgAb and IR which were statistically significant indicators as independent variables,and whether hypothyroidism as the dependent variable,the logistic regression of unconditional two-class classification was performed showed that IR and the serum IGF-1 were the influencing factors of diabetes mellitus complicated with hypothyroidism.4 The diabetes mellitus complicated with the thyroid nodule group suffered from age,duration of diabetes,FPG and TgAb were higher than the simple diabetes mellitus group,There was no significant difference in the serum IGF-1 level between the two groups.The relationship between the serum IGF-1 and TI-RADS grade of thyroid nodules was analyzed by Spearman correlation analysis,The results showed that there was a positive correlation between serum IGF-1 and TI-RADS grade of thyroid nodules?rs=0.524,P=0.001?.Taking age,duration of diabetes,FPG and TgAb as independent variables,and whether thyroid nodule as the dependent variable,the logistic regression of unconditional two-class classification was performed showed that the serum IGF-1 was not the influencing factors of diabetes mellitus complicated with thyroid nodule.duration of diabetes,FPG and TgAb were the influencing factors of diabetes mellitus complicated with thyroid nodule.Conclusions1 The serum IGF-1 level was correlated with diabetes mellitus complicated with thyroid disease,The serum IGF-1 level in patients with diabetes mellitus complicated with thyroid disease was lower than that in patients with simple diabetes mellitus.2 The serum IGF-1 level in patients with diabetes mellitus complicated with hyperthyrea was lower than that in patients with simple diabetes mellitus.3 The serum
