Object:Collect basic clinical information of hospitalized patients with acute myocardial infarction in a top three hospital of the Corps,analyze its clinical characteristics,understand the use of related drugs,and analyze the influencing factors of drug use,so as to better guide clinical standardized drugs.Methods:To collect information on the basic conditions,auxiliary examination,clinical diagnosis,coronary angiography results,drug use and other information of 1322 patients with acute myocardial infarction admitted from January 2016 to January 2018;analyze their clinical characteristics,drug use rate and The factors affecting drug use were analyzed by single factor and multivariate logistic regression analysis,respectively.Results:1.General information and clinical features:1322 patients were enrolled,the age of onset was61.59±13.31 years old,males were more than females?76.6%vs.23.4%?;the type of myocardial infarction was mainly STEMI?71.2%?,The anterior wall and inferior wall of the myocardial infarction area were the main?47.7%,37.7%?;The incidence rate is higher in autumn and winter,The proportion of smoking,overweight or obesity,hypertension,and diabetes was higher,45.8%,67.7%,53.9%,and 31.2%,respectively;The number of coronary lesions was mainly double and multiple?28.3%,45.7%?,the in-hospital mortality was 10.1%,and heart failure,atrial fibrillation,and cardiogenic shock were 21.9%,7.5%,and 6.6%,respectively.2.Analysis of treatment medication:Emergency reperfusion was 39.3%,of which emergency PCI was 36.3%,thrombolysis was 2.8%;Oral drug use:during hospital/discharge with medication?use rate?:dual antiplatelet drugs:94.6%/88.6%,single antiplatelet drugs:2.9%/6.1%,of which aspirin 93.3%/89.1%,P2Y12 affected Body antagonists 96.4%/92.3%,statins 96.0%/93.1%,ACEI/ARB52.4%/53.7%,beta blockers 66.6%/62.5%,combined with 5 drugs 41.0%/37.3%;Dosage:anti-platelet drugs:aspirin,indobufen,ticagrelor dose compliance rate of 100%,clopidogrel dose compliance rate:during hospital/discharge with drugs?99.3%/99.2%?;The low-intensity,moderate-intensity,and high-intensity use rates of statins were 47.5%,50.9%,and 1.6%,respectively;the doses of fosinopril and benazepril in the ACE inhibitors were higher than those in the hospital?8.56±2.770mg/dvs.7.85±2.930mg/d,8.67±3.287mg/dvs.7.82±3.185mg/d?,metoprolol sustained-release tablets and bisoprolol discharge zone in?-blockers The dose was higher than that during the hospital?40.47±14.023mg/dvs.35.22±14.027mg/d,3.65±1.410mg/dvs.3.22±1.385mg/d?,and the difference between the above groups was statistically significant?P<0.05?.3.Analysis of influencing factors of drug use:Logistic regression analysis showed that the factors affecting drug use mainly include the title of the patient's doctor,the contraindications for drug use,adverse drug reactions,age,myocardial infarction,admission to specialist departments,and hospitalization.Whether to PCI or so.Conclusion:Acute myocardial infarction is mainly in middle-aged and elderly patients,with more males,often accompanied by a variety of risk factors and chronic diseases,high in-hospital mortality,and more complications;The use of drugs related to acute myocardial infarction is not standardized,and there is a large gap with the recommended drugs in the guidelines;Factors affecting the use of drugs may be related to the title of the competent physician,the contraindications to the use of the drug,adverse drug reactions,age,location of the myocardial infarction,admission to the specialist department,and whether or not PCI is performed in the hospital.
