BackgroundThe rapid growth of hospital expense has become a common problem in the world. As the largest developing country, China also pay attention to the problem of hospital expense rising too fast and resource wasting, the government for the people " the doctor in a difficult and expensive " problem has taken a series of effective measures, increase of China’s medical and health system reform pace, but still need to seek a need to further improve the link. The fourth national health services survey data show the proportion without medical phenomenon in our country is larger because of economic difficulties or expensive treating. And the excessive growth of hospital expense have brought huge loss to peoples’health and economic life. How to control the unreasonable increase of hospital expense has become the important problem demanding prompt solution in medical industry.Objectives of the researchThis article research the demographics and hospital expense of the acute myocardial infarction patient in2006-2010years, then analysis of hospitalization expense proportions and identification the components’influence on the total hospitalization expense. Put forward the control measures and suggestions on costs control for the hospital, the management of single disease cost provide new train of thought, for medical management and medical insurance institutions to develop single disease payment standard provides data support.MethodsThis research based on acute myocardial infarction of constitution of hospitalization expense and influence factors. Using a retrospective survey research method, collecting a3A hospital2006-2010years of hospitalization for acute myocardial infarction patient medical records and hospital costs associated with reference data, the diagnosis of acute myocardial infarction basis and standard into the case, received a total of5years a case324. With all data in EXCEL database and proofreading, SPSS17.0were used to describe demographic data, hospitalization expenses, basic composition, at the same time using grey relational analysis(GRA) determine the cost breakdown on the total hospitalization expenses of influencing degree.Main ResultsThis study shows patients with acute myocardial infarction mostly are elderly male, but its age distribution reveals the patients presenting with young trend, all patients length of stay decreased. Operation patients’length of stay is longer than that of non-operation patients.In patients with acute myocardial infarction, hospitalization expenses distribution display, operation treatment of hospital costs in patients was higher than that of non operation treatment of patients. Simultaneous analysis of the operation and non operation patients hospitalization expenses can be found, operation patients hospitalization expenses each year basically presents downtrend Institute, and non operation patients hospitalization expenses each year basically presents the trend of escalation.All hospitalized patients hospitalization expenses, the cost of materials and medicine accounted for the highest proportion of material cost and expenses; the opposite change tendency; operation and non operation patients hospitalization expenses of patients with operation, cost of materials, and non operation patients with drugs mainly. Using gray correlation analysis to judge each part of the total cost of hospitalization cost influence, operation patients in bed, medical fees, inspection fees for maximum impact, non operation patients in material costs, care, treatment cost on the total cost of hospitalization impact is ranked in the top three, operation group a larger proportion of the cost of materials and non the largest proportion of the cost of operation group influence degree are small.ConclusionsGender, age, payment methods, treatment methods and other factors are one of the important factors affecting hospitalization expenses. Clinical path management work can be able to more standardized treatment services, control hospital charge. The cost of materials and drugs is the highest proportion of total cost of hospitalization, hospitalization costs increases with the material cost and drug cost. Vacant management regulation and low input also are the important factors to promote the increase of hospitalization expenses. The cost of materials, the largest proportion of hospitalization expenses with operation treatment of patients and drug cost, the largest proportion of the total cost of hospitalization expenses with non operation treatment of patients are relatively stable, so bed fees, medical fees, examination fees become the greatest influence in the cost of operation hospitalization, the influence degree of material costs, care cost, treatment cost is ranked in the top three on the total hospitalization costs of non-operation hospitalization.
