The Clinical Retrospective Study Of The Comprehensive Treatments Of Vascular Anomalies With Propranolol

Objective : A retrospective summary about the application and distribution of propranolol in the multiple treatments of vascular anomalies.To analyze and compare the difference of curative effect and safety evaluation between different treatment methods and pathological types.Methods:We collected the total number of 256 cases of vascular anomalies who were confirmed in the Department of Surgical oncology,Children's hospital affiliated to Chongqing medical university,from December 2016 to December 2018 and after exclusion standard screening a total of 137 cases included in the study.The treatment groups can be divided into three : group 1 took propranolol orally only,group 2 took propranolol combined with local injection treatment,group 3 took propranolol combined with other comprehensive treatment.Recorded the change of the lesion before and after treatment,such as the size,color,accompanied symptoms and so on.We evaluate the curative effect according to a four-classification criteria created by Achauer.Results:1.In treatment group 1,the treatment effective rate of vascular tumor and vascular malformation was 80.65% and 66.67% respectively,the curative effect in the three types of infantile hemangioma was no statistically significant difference distribution.2.In treatment group 2,the effective rate of infantile hemangioma was 87.80%,and there was no statistically significant difference distribution of the curative effect in its three types;The effective rate of vascular malformation was 75.68%,and there was no statistically significant difference distribution of the curative effect in its four types.3.In treatment group 3,the treatment effective rate of vascular tumor and vascular malformation was 92.31% and 88.89% respectively.4.The treatment effective rate of vascular tumor in general was 85.88%,which was higher than the rate of 76.92% in vascular malformation.5.The effective rate of a single hair type of infantile hemangioma in the treatment group 1 and 2 was 85% and 90% respectively,there was no statistically significant difference,but the overall treatment time of the treatment group 2 is shorter than the other,and the difference was statistically significant.The Incidence of abnormal liver function had no significant difference between two treatment groups.Conclusion:The curative effect of propranolol has been widely recognized as the first-line treatment of infantile hemangioma.On this basis we got a step futher and found it may have good curative effect on vascular malformation too.2.The therapy effective can be improved compared to propranolol only if it joint with other comprehensive methods in the treatment of complex or refractory vascular anomalies.3.The single hair type of infantile hemangioma can get good results both in the treatment group 1 an group 2,but the latter can reduce the overall course of treatment,and did not increase the incidence of adverse reactions.
