Diagnosis And Treatment Of Pregnancy Complicated With Adrenal Tumor

Objective:Based on the analysis of the clinical data of 5 patients with adrenal tumor during pregnancy and literature review,the etiology,clinical characteristics,diagnosis and treatment and prognosis of adrenal tumor during pregnancy were discussed to improve the diagnosis and treatment,reduce the missed diagnosis and misdiagnosis rate,and improve the survival rate of pregnant women and fetuses.Method:The clinical data of 5 cases of pregnancy complicated with adrenal tumor admitted to the first affiliated hospital of chongqing medical university from January 2015 to September 2018 were retrospectively analyzed.Meanwhile,literature materials were reviewed and combined with literature to discuss the diagnosis,diagnosis and treatment of pregnancy complicated with adrenal tumor.Results:The data collected included 5 cases of adrenal tumor associated with pregnancy,aged 23-37 years.The first clinical symptoms of all the 5 patients were increased blood pressure.Among them,2 cases occurred in the second trimester and 3 cases occurred in the third trimester.One patient was pregnant with Cushing's syndrome,one with primaryaldosteronism,one with nonfunctional adenoma,and two with pheochromocytoma.Three patients in the third trimester all gave birth to healthy baby by cesarean section,and two patients in the second trimester all had miscarriages.Laparoscopic adrenal tumor resection was performed in 3 patients after abortion or delivery.Four pregnant women and two infants were followed up with good health,and one patient with pheochromocytoma were lost to follow-up.Conclusion:Adrenal tumors associated with pregnancy are very rare in clinical practice,with diverse and poor specificity in symptoms and signs,making diagnosis difficult.Once missed diagnosis or misdiagnosis will bring great harm to pregnant women and fetuses,different treatment options should be selected according to the pregnancy stage,tumor type and patients' personal preference.
