Objective Analysis of long-term patency rate and related influencing factors of vascular injury in lower limbs treated with stent-graft.Methods A retrospective analysis was performed on patients with vascular injury of lower extremity who underwent stent-grafting in our hospital from January 2012 to March 18,18,except for 10 patients who were lost.A total of 116 patients(116 limbs)were obtained.According to the follow-up results,they were divided into an open group and an occlusion group.Statistical analysis was performed on the cumulative patency rate at 1 year,3 years,and 5 years after operation and the related factors affecting postoperative stent occlusion.The relevant influencing factors included gender,age,cross-articular,infection,stent type,stent diameter,and stent length.Binary logistic multivariate regression analysis was performed if the correlation factor P value was <0.1.Results There were 116 patients in the first year,92 patients in the third year,and 64 patients in the fifth year.The cumulative patency rates at1 year,3 years,and 5 years after surgery were 80.2%,66.2%,and 56.9%,respectively.The cumulative patency rates of the second phase were 95.0%,82.3%,and 62.7%,respectively.Compared with the unobstructed group,the age of the occlusion group,the length of the stent,the female,the Fluency stent,the infection,and the cross-arthrosis ratio increased within 1year after surgery,and the stent diameter,male,and Viabahn stent ratio decreased;the stent length of the occlusion group within 3 years after surgery,Fluency stent,infection,cross-articular ratio increased,stent diameter,Viabahn stent ratio decreased,the difference was statistically significant(P < 0.1).Multivariate analysis showed infection(OR=4.614,95% CI: 1.233 ~ 17.267),cross-articular(OR=5.954,95% CI: 1.600 ~22.159),stent length(OR=1.046,95% CI: 1.005 ~ 1.089)It was an independent risk factor for stent occlusion within 1 year after operation.The stent diameter(OR=0.495,95% CI: 0.312-0.787)was the protective factor for stent occlusion within 1 year after surgery;cross-articular(OR=6.353,95% CI: 1.565)~25.800),stent length(OR=1.043,95% CI:1.001 to 1.086)is an independent risk factor for stent occlusion within 3years after surgery.Conclusion The stent graft is effective in the treatment of lower extremity vascular injury.Infection,cross-articular and stent length are independent risk factors for postoperative patency,and larger stent diameter is a protective factor.
