Background:With social modernization,trauma incidence is rising up.Some severe contaminated and infectious wounds are the thorny problems needing to be resolved in surgical wound treatment.VSD(Vacuum Closed Drainage)has been recognized by many doctors since it was proposed and applied clinically in 1993.However,VSD also has obvious drawbacks such as the high-cost VSD material,hardening foam material after being used for some time,the obstruction of the drainage pipeline requiring to be replaced many times,which has increased the economic burden of the patients.Moreover,the contaminated secretions in foam materials and wounds would exacerbate the wound infection if the VSD is not replaced in time.Continuous normal saline irrigation is added to improve VSD in order to observe whether this combination could increase the service life of negative pressure drainage device and improve the therapeutic efficacy for severe contaminated wounds.Objective:To study and analyze whether VSD with continuous normal saline irrigation could improve the therapeutic efficacy and increase the service life of negative pressure drainage device.Methods: From Oct.2016 to Oct.2017,20 patients with severe contaminated wounds and 18 patients with infected wounds were treated with VSD as the 38 cases control group.From Aug.2017 to Dec.2018,23 patients with severe contaminated wounds and 20 patients with infected wounds were treated with VSD combined with continuous normal saline irrigation as the 43 cases experiment group.By systematically observing and collecting the wound healing,the time(d)available for second-stage repair,the positive rate of bacterial culture during treatment,the blockage rate of drainage tube during treatment,and the service life(d)of negative pressure drainage device in each treatment etc,the data were collected and statistically analyzed to explore the therapeutic efficacy of VSD with continuous normal saline irrigation.Result: Compared with VSD alone,there were statistically significant differences in the efficacy rate of wound treatment(65.8%:88.4%,P = 0.014),the positive rate of bacterial culture(47.4% : 23.3%,P = 0.020),the service life of negative pressure drainage device and the second-stage repair time(P < 0.05)between the two groups.Conclusion: In comparison with VSD,VSD combined with continuous irrigationcould shorten the wound healing time,reduce the wound infection probability and increase the service life of VSD,which showed better therapeutic efficacy and was worthy of extensive clinical application.
