Objective: To observe and evaluate the clinical application effect of VSD(Vacuum Sealing Drainage)combined with irrigation of Recombinant Bovine Basic Fibroblast Growth Factor(rb-bFGF)on wound,and to explore a new therapy to accelerate the healing time of wounds.Methods: 90 hospitalized patients who were selected from the Burn and Plastic Surgery Department of Affiliated Hospital of Northern Sichuan Medical University from January 2016 to December 2017 were randomly divided into 3 groups,each group of 30 cases.After gross observation was conducted and wound secretion was sent for bacterial culturing right after admission,and chose antibiotics according to the results.After wound debridement,group A was treated with VSD;group B was treated with VSD combined with irrigation of saline;group C was treated with VSD combined with irrigation of 2% rb-bFGF.The flushing group was to add a flush tube to the VSD dressing for irrigation.Each VSD maintained effective drainage for 5-7 days.Skin grafting was used to cover the wound after good growth of granulation tissue.The negative rate of bacterial culture,drainage tube blockage rate,the number of VSD,hospitalization days and other indicators of the three groups were observed and evaluated.Results:1.General situation: There was necrotic tissue and purulent secretions,and without granulation tissue or a little aged granulation tissue in the 3 groups of Patients' wound before debridement.After 7 days of VSD treatment,there was fresh granulation tissue growth,and both wound and wound edema were reduced obviously in the 3 groups of Patients' wound.In group C,a large amount of fresh granulation tissue was found on the basement of the wound,which was significantly more than that in group A and B.And there was no necrotic tissue and purulent secretion in group C.2.After 7 days of VSD treatment,the negative rate of bacterial culture in groups B(88.9%)and C(92.3%)were significantly higher than group A(64%),and the difference was statistically significant(P<0.05).The negative rate of bacterial culture in group C(92.3%)was higher than group B(88.9%),but there was no significant difference between the two groups(P>0.05).During the wound repair,the drainage tube blockage rate in groups B(2.13%)and C(2.38%)were significantly lower than group A(15.25%),and the difference was statistically significant(P<0.05).The drainage tube blockage rate in group C(2.38%)was slightly higher than group B(2.13%),but there was no significant difference between the two groups(P>0.05).3.During the wound repair,the number of VSD in groups B(1.57±0.63)and C was(1.40±0.50)was significantly less than group A(1.97±0.72),and the difference was statistically significant(P < 0.05).The number of VSD in group C(1.40±0.50)was less than group B(1.57±0.63),but there was no significant difference between the two groups(P>0.05).The hospitalization days in group C was(24.27±6.97)days less than groups A(35.87±10)days and B(33.73±8.75)days,with a significant difference(P <0.05).The number of hospital stays in group A(35.87±10.00)days was slightly longer than group B(33.73±8.75)days,with no statistically significant difference between the two groups(P>0.05).Conclusions:1.Both VSD combined with irrigation of recombinant bovine basic fibroblast growth factor(rb-bFGF)fluid and VSD combined with irrigation of saline can reduce the drainage tube blockage rate,the number of VSD,the wound infection,and increase the negative rate of bacterial culture compared to simple VSD in wound repair.2.VSD combined with irrigation of recombinant bovine basic fibroblast growth factor(rb-bFGF)fluid can significantly shorten the hospitalization days compared with simple VSD and VSD combined with irrigation of saline in wound repair,which may be related to recombinant bovine basic fibroblast growth factor(rb-bFGF)promoting wound healing.It is worthy of clinical popularization and application.
