Objective: To study the prevalence,demographic characteristics and the influencing factors correlates of mental disorders among female prisoners in jiangsu province.To identify high-riskgroups and to discuss the effective management mode for such special group under the current supervision conditions.Method:The census and stage screening were conducted between October 2015 and March 2016 for 2,454 female prisoners in a prison in jiangsu province(all female aged 16 and above).The International Classification of Diseases,ICD-10-AM and ICD-10,were used as the diagnostic tools and classification standards,with using a series of domestic and foreign self-rating scales and nurse-administered rating scales and combine with the diagnosis of a psychiatrist to diagnose the different types of mental disorders among female prisoners.The scales be used are as follows:? General Health Questionnaire ?(self-assessment)? Twelve Health Screening Items ?(self-assessment)?Suicidal Ideation Self-rating Scale?(Self-assessment with help from others)?Personality Diagnosis Questionnaire?(Self-assessment with help from others)?Mental Health Information Questionnaire?(Self-assessment with help from others)?A List of Clues to Mental Disorders?(written by the branched policemen)?Diagnostic information questionnaire for mental disorders?(written by the branched policemen)and?ICD-10-AM List of Symptoms of Mental Disorders?(written by the psychiatrist).EpiDate3.0 was used to entry data,data analysis mainly by using SPSS17.0.Logistic regression analysis was used to investigate the influence of demographic characteristics,the prior information,and the influence of environment on mental disorders,and to explore the current regulation conditions for such a effective management mode among female prisoners.The test level(alpha)was 0.05,and P<0.05 was considered the significance of difference existed.Results: According to the survey and the diagnosis of psychiatrists,107 of the 2,454 female prisoners serving in a prison in jiangsu province suffered from mental disorders,and the lifetime prevalence of mental disorders was 4.36%.The prevalences of various mental disorders are as follows: emotional disorders(40,1.63%);Schizophrenia and other psychiatric disorders(23,0.94%),neurosis(17,0.69%),personality disorder(10,0.41%),mental retardation(8,0.33%),organic mental disorders(2,0.08%),mental disorders caused by psychoactive substances(2,0.08%);stress related disorder(2,0.08%);Psychological factors related to physiological disorders(1,0.04%);cultural related mental disorders(1,0.04%).The comparison of general demographic characteristics of the health group and the mental disorder group showed that there were statistically significant differences in age,culture and case(P<0.05).The comparison of the past medical history between the two group found that mental disorder group was significantly higher than healthy group in previous prevalence,the significance of difference existed(40.19% VS 19.43%,x~2=27.22,P<0.001),and as same as the previous history of self-injury and suicide(14.02% VS 4.56%,x~2=19.38,P<0.001).Besides,comparing the population of the prison discovered that mental disorder group has more old and disabled person(29.91% VS 7.97%,x~2=60.63,P<0.001),and has less manual work then healthy group(83.18% VS 92.67%,x~2=12.91,P<0.001).There has no difference between the two groups of distribution in smoking,drinking,taking drugs,and causing trouble(P>0.05),and also has no difference between the two groups of constitutions in family support,like family meeting,communication,ect(P>0.05).Seven factors,which proved to be different through single factor analysis was included in the multiple factors regression analysis model.The results showed that property crimes are less dangerous than violent criminal of mental disorders(OR=0.52,95% CI(0.29-0.94)).The history of somatic disease,self-injury and suicide is a risk factor for mental disorders(OR=2.01(1.74-3.44),OR=3.18(1.68-6.05)).The risk of mental disorders in supervision posts was reduced(OR=0.31(0.14-0.71)compared with manual work posts.Mental disorders(OR=10.01(4.71-21.14)are more likely to occur in the old and the disabled person.Conclusion: The prevalence of mental disorder of female prisoners in a female prison in jiangsu province is low.The first four mental disorders in female prisoners disease spectrum were affective disorder,schizophrenia,neurosis and personality disorder.Cases,past self-injury and suicide history,somatic diseases,labor posts and the area of the prison are the factors that affect the occurrence of mental disorders of female prisoners,which need to be focused on these factors;In daily supervision work,special attention should be paid to criminals with violent crimes,history of self-injury and suicide,somatic diseases and manual work positions;To strengthen the attention to female prisoners in the senile and infirm ward;The prevention for female prisoners of self-injury and suicide has gradually become a new task faced by the prison.
