Objective:to compare the clinical outcomes of standard lymphadenectomy(SLD)and extended lymphadenectomy(ELD)for ductal adenocarcinoma of the body and tail of the pancreas by robotic surgical system.Methods:Fifty-four patients with the pancreatic ductal adenocarcinoma of distal pancreas were analyzed retrospectively from April 2010 to September 2015 including 37 patients of SLD and 17 patients of ELD operated by robotic surgical system.Patient characteristics,blood loss,operative time,length of postoperative stay,postoperative complications and median survival time were compared.Results:The demographic data were similar between SLD and ELD.There was a significant difference of operative time between SLD and ELD(231.47 min vs 141.35 min,P<0.001).The patients in ELD group had longer length of postoperative stay than the patients in SLD group(22.76 d vs 20.40 d,P=0.009),while blood loss,pancreatic fistula,delayed gastric emptying,postpancreatectomy hemorrhage,postoperative infection,median survival time between 2 groups were similar(P>0.05)?Conclusions:ELD by robotic surgery system is safe,and will not increase the rate of complications There was no significant difference of efficacy between ELD and SLD.
