With the rapid development of China’s economy and the continuous improvement of people’s quality of life,people’s healthcare needs have been greatly released,and showing a variety of characteristics.However,the shortage of healthcare resources at the same time there are still such as unreasonable allocation of medical resources and many other criticisms.Although the medical system reform has been advancing in depth to strengthen the construction of community medical institutions,hoping to guide residents that get ill to choose the community medical institutions,but the residents’ hospital selecting behavior is not change in spite of the construction of hierarchical medical system has changed significantly,on the contrary grade 3 hospitals in the medical market has become increasingly prominent advantages,the proportion of medical treatment also increased year by year,"difficult to see a doctor" problem is more serious.[Objectives] The study takes Shanghai as an example,and tries to analyze the difficulties in the implementation of the hierarchical medical system polices.At the same time,it aims to use the Best-Worst Scaling method to analysis the selection behavior of Shanghai residents for medical institutions and to identify the most important factors that affect the behavior.Hoping to provide suggestions for the progress of the hierarchical medical system policy from the perspective of demanders,to meet the healthcare needs of residents,and improve the hospital selecting behavior,so that residents voluntarily follow the order of medical treatment.[Methods] Using the literature analysis method to collate the domestic and foreign researchs and related theories of residents ’choice of medical institutions,and designs the theoretical framework and research path suitable for this study in the light of the present situation of Shanghai residents’ choice of medical institutions.The questionnaire that used to obtain the data,was designed based on the Best-Worst Scaling method.The data were collected from a survey of 260 Shanghai residents in July 2017.The survey data were statistically analyzed by descriptive statistical analysis,Chi-square test,BWS analysis and conditional logit regression analysis,to analyze Shanghai residents’ hospital selecting behavior,willing to visit community health service center and the key factors that influence the behavior.[Results] The results show that: Currently,Shanghai residents still prefer to choose grade 3 hospitals,and there is excessive medical needs.In terms of individual characteristics,age,marital status,work status,family income status,type of insurance and the state of health will have a significant impact on residents’ choice of medical institutions.With regard to the selection tendency of the characteristics of medical institutions,residents are most concerned about the characteristics related to medical quality such as specialists,examination and treatment measures.Compared to the distance and high medical costs,people are more unable to accept less kinds of drugs and treatment measures.In view of the current allocation of medical resources and residents’ medical needs and preferences,puts forward some suggestions: 1.Clear the different levels of medical institutions position.2.Enhance the trust of residents about general practitioners.3.Liberalize primary health care institutions with drug restrictions.4.Strengthen the advantages of primary health care institutions.5.Promote the construction of medical association.
