Primary iliopsoas abscess(IPA),an uncommon clinical one of entity,is often nospecific clinical features which may adopt advance imaging techniques for diagnosis.The purpose of this study was to present 4 rare cases of primary iliopsoas abscess with rapid septic shock.From October 2016 to February 2017,four patients with primary iliopsoas abscess complicated with rapid septic shock were admitted to our departmen.In our cases,all patients were in shock at the time of admission and diagnosed with primary IPA by history,clinical examination,imageological diagnose and microbiological test.All cases were treated by incision drainage and sensitive antibiotic.The patients eventually recovered and discharged within 2 months.During the 6-month follow-up,3 patients had normal life,no pain in the waist and abdomen,and the hip joint was mobile.1 patient was admitted to hospital with recurrence of abscess and underwent incision and drainage again.45 days later,she was completely recovered and discharged.Although there have been reports of primary IPA in previous literature,the four cases reported in this paper were different from the previous literature reports in terms of the scope of the infected area and the size of the abscess.Since primary iliopsoas abscess does not have specific clinical symptom or sign,cannot diagnose in time.Therefore,special attention must be paid to patients with sudden abdominal pain,hip pain or high fever without obvious causes,which avoid missed diagnosis and misdiagnosis.
