Objective: To investigate the relationship between specific pelvic fracture types and posterior urethral injury by retrospectively analyzing the cases of male pelvic fracture combined with posterior urethral injury in our hospital for 10 years.Materials and Methods: 788 cases of pelvic fractures were reviewed,including 77 cases with posterior urethral injury.There were fractures of sacrum(182 cases),fractures of ilium(304 cases),fractures of acetabular(236 cases),pubic symphysis separations(150 cases),and ankle joints(128 cases).According to the number of fractures of pubic ramus,cases were divided into fractures of 1 pubic ramus(100 cases),fractures of 2 pubic ramus(381 cases),fractures of 3 pubic ramus(46 cases),fractures of 4 pubic ramus(165 cases),and analyses the relationship between the occurrence of urethral injury and the location and type of pelvic fracture according to Tile classification.Results: The total incidence of pelvic fractures with posterior urethral injury in male patients was 9.8%.The incidence of posterior urethral injury(22.4%)of 4 fractures of pubic ramus was significantly higher than that of 1?2 and 3 fractures(P<0.05);The incidence of posterior urethral injury in separation of the symphysis pubis(18.0%)was significantly higher compared with the unseparated of the symphysis pubis(7.8%)(P<0.05);The incidence of posterior urethral injury in the dislocation of sacroiliac joint(15.8%)was significantly higher compared with the unseparated of the dislocation of sacroiliac joint(8.7%)(P<0.05);There was no significant difference in the incidence of posterior urethral injury between the fractures of sacrum,ilium and the acetabular.C-type pelvic fractures in the Tile classification were significantly associated with the posterior urethral injury(P<0.05).It is indicates that the more severe the pelvic fracture occurs,the more likely posterior urethral injury would happen.Conclusions: In male patients with pelvic fractures,multiple pubic symphysis fractures,pubic symphysis separation,and dislocation of sacroiliac joint may increase the posterior urethral injury.At the same time,In the Tile classification,type C pelvic fractures are more prone to posterior urethral injury.
