Objective: To investigate the relationship between MTHFR C677 T polymorphism and Hcy and the occurrence,development and prognosis of cerebral infarction with atrial fibrillation.Methods:From November 2017 to November 2018,102 patients with atrial fibrillation cerebral infarction diagnosed in the Department of Neurology,Second Hospital of Hebei Medical University were selected as the case group,and 90 healthy subjects were used as the control group.The fasting venous blood was directly preserved for the detection of gene polymorphism.The case group was divided into the cerebral embolism group and the cerebral infarction group according to the clinical symptoms,imaging examination and the 2010 European echocardiographic guidelines.Complete data of case group and control group were collected,and the modified Rankin Scale(MRS)score was followed up for 90 days after discharge.MRS ? 2 was recorded as good prognosis group,and MRS ? 3 score was recorded as poor prognosis group.Data processing was performed using SPSS 21.0 statistical software.Result:1.There was no significant difference in the frequency of MTHFR genotype distribution between the case group and the control group(P <0.05).Plasma Hcy levels were closely associated with the development of cerebral infarction with atrial fibrillation(t=4.501,P =0.000).2.MTHFR gene polymorphism was positively correlated with Hcy level,and was not directly related to cerebral infarction with atrial fibrillation.3.Multivariate logistic regression analysis pointed that: NIHSS score,CHA2DS2-VASc score,and anticoagulant therapy were independent risk factors for the prognosis of patients with cerebral infarction(P <0.05).Conclusion:The level of homocysteine in the acute phase of the disease is closely related to the occurrence of patients with cerebral infarction with atrial fibrillation.There was no direct correlation between MTHFR gene polymorphism and cerebral infarction with atrial fibrillation,and it was closely related to plasma homocysteine level in acute stage.MTHFR gene polymorphism is indirectly involved in the occurrence of cerebral infarction with atrial fibrillation,possibly through homocysteine.NIHSS score,CHA2DS2-VASc score,and oral anticoagulant therapy after discharge were independent risk factors for short-term prognosis in patients with cerebral infarction.
