The Effects Of Nalbuphine On Preventing Occurrence Of Myoclonus Induced By Etomidate In Patients Undergoing EVL

Objective:To investigate the effects of Nalbuphine and Sufentanil on preventing occurrence of myoclonus induced by etomidate and whether they would affect stability of respiration and circulation in patients undergoing esophageal gastric varix ligation operation.Methods:A total of 40 patients requesting for esophageal gastric varix ligation operation were selected,ASA II-III,the age at 45-70.All them were randomly divided into Nalbuphine group and Sufentanil group,20 cases in each group.Before the anesthesia induction,two groups were respectively pretreated with Nalbuphine 0.1mg/kg and sufentanil 0.1ug/kg.At 2 min after injection,etomidate0.3mg/kg was administered intravenously with 20s.The occurrence and severity of myoclonus etomidate-induced were assessed and recorded with 2 min after injection of etomidate.The vital signs and side-effects were monitored.Result:1.There was no significant difference about general condition between the two groups?P>0.05?.2.The occurrence of myoclonus was 10%and 15%in Nalbuphine group and Sufentanil group respectively.There was no significant difference between the two groups?P>0.05?.3.At the time point of T4,the heart rate and blood pressure in Sufentanil group were significantly higher than those in Nalbuphine group?P<0.05?.At the time point of T5,the blood pressure in Sufentanil group was significantly lower than those in Nalbuphine group?P<0.05?.4.Total dosage of propofol in Sufentanil group was significantly higher than that in Nalbuphine group?P<0.05?.5.The occurrence of severe body movement in Sufentanil group was significantly higher than those in Nalbuphine group and the difference was statistically significant?P<0.05?.6.There was no significant difference in operation time,anesthesia recovery time,SpO₂<90%respiratory depression,nausea and vomiting,SpO₂and HR,MAP at other time between the two groups.Conclusion:Both of Nabuphine and sufentanil have good preventive effect on myoclonus induced by etomidate.Nalbuphine has less influence on circulation and respiratory system,less side-effects and better analgesic effect.It is more suitable for patients undergoing esophageal gastric varix ligation operation under general anesthesia.
