Objective: Acute pancreatitis in pregnancy(APIP)is a critical obstetric disease with a low incidence,but it's condition is progressing rapidly.It markedly threatens maternal and fetal health.Through the retrospective analysis,this study aimed to describe the pathogenic factors,clinical manifestations,treatment options and pregnancy outcomes.Therefore,we can summarize the clinical characteristics,strengthen the understanding of acute pancreatitis in pregnancy(APIP),in order to improve the pregnancy outcome.Methods: This study retrospectively analyse the clinical data of 72 patie-nts of acute pancreatitis in pregnancy from January 2008 to August 2018 admited to the Second Hospital of Hebei Medical University.Result: The incidence of the APIP is on the rise,adipogennic factors increase year by year in the proportion of etiological factors.Among the 72 patients with acute pancreatitis in pregnancy,the most etiological of APIP are the lipogenic factor(30 cases,41.7%)and idiopathic factor(30 cases,41.7%),other factors include 12 cases of biliary factors(16.7%).There were significant differences in the severity of the disease caused by different etiology(P<0.05),lipogenic factor is more likely to result in severe acute pancreatitis(P<0.05).There were statistically different causes of albumin value,blood calcium and blood glucose in the three groups(P<0.05),the adipogenic APIP is more likely result in hypocalcemia and hyperglycaemia.In the three different etiological factors,there were no significant differences in the mode of termination of pregnancy the rate of pretermbirth,the rate of the incidence of fetal distress,the incidence of neonatal asphyxia and mortality(P<0.05).Conclusion:1.The incidence of the APIP is on the rise,adipogennic factors increase year by year in the proportion of etiological factors.2.Adipogenic factor and idiopathic factor are the main causes of APIP,and the adipogenic facors are more likely to result in severe acute pancreatitis.3.The adipogenic APIP is more likely result in hypocalcemia and hyperglycaemia,leading to adverse pregnancy outcomes.4.The treatment of APIP should take into account both maternal and fetal situation,according to the condition,etiology and gestational weeks.In order to make the personalized treatment,choose the appropriate timefor operation and termination of pregnancy.
