Objective:This paper explored the changes of serum TNF-? level in people with different glucose tolerance,and analyzed its correlation with glucose metabolism parameters,lipid metabolism parameters,insulin resistance,Visceral adipose tissue area,etc.,to explore its significance in early hyperglycemia and its possible mechanism.Methods:In Shijiazhuang communities,the total 328 subjects aged between 18 and 70 years old who met the criteria of entry and discharge,were recruited and signed the informed consent voluntarily and divided into normal glucose regulation(NGR)group 118,impaired glucose regulation(IGR)123 and newly diagnosed type 2 diabetes group(T2DM)87 by OGTT experiment.Collect gender,age,past history and other information of all subjects;Measure height,weight,waist circumference,hip circumference and other indicators;body mass index(BMI)and waist-to-hip ratio(WHR)were calculated.Detection of liver function,renal function,blood lipids and other biochemical indicators,0.5 hours blood glucose,2 hours blood glucose,fasting insulin,0.5 hours insulin,2 hours insulin,Glycated hemoglobin;calculation of HOMA IR,HOMA-beta and glycosylated hemoglobin sugar metabolism related indicators,with dual-energy X-ray detection body fat percentage,Visceral adipose tissue area,relevant data such as body fat men fat percentage,and with ELISA method to detect the serum level of TNF-alpha.Results:1.There were no statistically significant differences in gender,age,blood pressure,smoking history,drinking history,previous history,family history and general biochemistry: liver and kidney function,blood lipid index among the three groups(P> 0.05);2.Using the single factor analysis of variance NGR,IGR,T2 DM between the three groups of FBG,0.5 h blood glucose,fasting glucose 2 h blood glucose,glycosylated hemoglobin have a statistically significant difference(P < 0.001),compared with NGR group,IGR and T2 DM group of FBG,0.5 h blood glucose and 2 h a significant rise in blood sugar,glycosylated hemoglobin,compared with IGR group,T2 DM group of FBG,0.5 h blood glucose and 2 h elevated blood sugar,glycosylated hemoglobin,the difference was statistically significant(P< 0.05).Compared with NGR group,homa-ir of IGR group and T2 DM group showed statistical difference(P < 0.001),while that of IGR group showed no statistical difference(p > 0.05).Compared with NGR,there was no significant difference between IGR group(P > 0.05)and T2 DM group(P< 0.001).Compared with IGR group,T2 DM group decreased,and the difference was statistically significant(P < 0.001).3.Compared with the NGR group,the IGR and T2 DM groups had statistically significant differences in visceral adipose tissue area(VAT),waist circumference,waist-to-hip ratio,and body mass index(BMI)(P<0.05),but there was no comparison between IGR and T2 DM groups.Significant difference.Compared with the NGR group,the IGR group increased in the weight-to-weight ratio of obesity and the percentage of male fat(A),and the difference was statistically significant(P<0.05).There were no significant differences in body fat percentage,percentage of peripheral fat,and percentage of female fat in the three groups(P>0.05).The levels of TNF-? in the three groups were statistically different(P<0.001).TNF-?levels were significantly higher in the IGR group and T2 DM group than in the NGR group(P< 0.001).TNF-levels were also significantly higher in the T2 DM group than in the IGR group(P < 0.001).Pearson correlation analysis showed that TNF-? was positively correlated with male fat percentage,Visceral adipose tissue area,HbA1 C,HOMA-IR,and body fat percentage,with statistical significance(P< 0.05).Multivariate regression analysis showed that Visceral adipose tissue area was an independent risk factor for TNF-? elevation(P=0.007).Conclusion:1.The serum TNF-? level of the pre-diabetic population was higher than that of the normal population,indicating the existence of inflammatory reaction in the pre-diabetic population.With the increase of blood glucose,the TNF-? level was further increased,and the inflammatory reaction in the body was further aggravated.2.The visceral fat area increased in the IGR group and T2 DM group,suggesting that the increase in visceral fat was the main cause of early blood glucose elevation.3.With the increase of Visceral adipose tissue area,the serum TNF-? level gradually increased.Multiple regression analysis showed that Visceral adipose tissue area was an independent risk factor of TNF-?,suggesting that the increase of Visceral adipose tissue area led to the aggravation of inflammatory reaction in the body,resulting in the increase of TNF-? level.4.With the increase of blood glucose,serum TNF-? level increased and insulin resistance increased,which showed the same increasing trend as TNF-?.It was considered that the increased TNF-? might further cause blood glucose abnormalities by aggravating insulin resistance.
