Objective: The clinical features and related factors of adverse prognosis of 379 patients with thyroid cancer treated in our hospital from 2002 to 2013 were analyzed.Methods: A retrospective survey of 379 patients(including 45 thyroid follicular carcinomas and 334 thyroid papillary carcinomas)who underwent thyroid cancer treatment in our department over the past 12 years,obtained gender,age,maximum tumor diameter,and lymph node Clinical data such as metastasis,extra-glandal infiltration,and influencing factors of poor prognosis according to the follow-up database of the department and new follow-up patients.Result: The 379 patients with thyroid cancer who were treated in our Department included male and female patients,and the number of cases was 116 and 263,respectively.Its male to female ratio is 1:2.3.The youngest is only 8 years old,the highest is 85 years old,and the average age is 44.34±14.34 years old.Among the 379 patients,12 patients and 31 patients died and relapsed.Log-rank univariate analysis found that age,tumor diameter,lymphatic metastasis,and(American Joint Committee on Cancer,AJCC)staging were the influencing factors of thyroid cancer-specific survival(Cancer-specific survival,DSS).Age(55-year-old cut-off point),tumor maximum diameter,lymphatic metastasis,and AJCC staging were High risk factors for disease-free survival(Disease-free survival,DFS)in thyroid cancer.DSS-related multivariate Cox regression analysis of thyroid cancer found that the risk of death in stage IV patients was OR=104.537(95% CI: 18.804,581.567;P<0.0001)compared with the risk of death in stage I,II,and III patients.Multivariate Cox regression analysis of thyroid cancer with DFS found that lymph node metastasis and AJCC staging are independent risk factors for total adverse prognosis in patients with thyroid cancer.The risk of adverse events in patients with lymph node metastasis is associated with the risk of adverse prognosis in patients without lymph node metastasis.The odds ratio for OR=4.569(95% CI: 1.022,20.664;P=0.0047),the risk of total adverse prognosis in stage IV patients was 136.669 times that of stage I,II,and III patients(95% CI: 37.752,494.979;P<0.0001).Conclusion:AJCC staging is an independent influencing factor for death endpoints in thyroid cancer patients,and independent risk factors for adverse prognosis are lymph node metastasis and AJCC staging.
