OBJECTIVE: To investigate the plasma levels of calcitonin gene-related peptide and endothelin in the episodes of migraine and episodes,and to explore the differences and associations between migraine patients with and without right-to-left shunt.Analyze the pathogenesis of migraine.METHODS: Eighty-five patients were diagnosed as having "aura with migraine" from November 2017 to December 2018 in the Department of Neurology,the First Affiliated Hospital of Jiamusi University,including 28 male patients and 57 female patients,age 18-55 years old(34.64 ± 9.21 years old),the group was consistent with the diagnosis of aura migraine in accordance with the 2013 IHS International Headache Classification Third Edition.The patients included in the experiment underwent intracranial vascular color Doppler ultrasonography and foaming test.According to the results of the examination,the patients were divided into right and left shunts.85 migraine patients were divided into migraine patients with RLS.(RLS group)and the migraine patient group(non-RLS group)in the absence of RLS,the patients were divided into the attack phase(A phase)and the interictal phase(I phase)according to the onset time.At the same time,30 healthy subjects in the same period were selected as the healthy group,including 12 males and 18 females,aged 19-50 years(36.67±6.79 years old).The levels of calcitonin gene-related peptide and endothelin in peripheral blood of all enrollees were determined.result:1.The levels of calcitonin gene-related peptide and endothelin in peripheral blood of patients with migraine were significantly increased in the episode,and there was statistical difference between them and healthy controls(p<0.01).2.The content of calcitonin gene-related peptide and endothelin in peripheral blood of patients with migraine increased in the interictal period,and there was statistical difference between the two groups(p<0.05).In the interval of migraine attack,the levels of calcitonin gene-related peptide and endothelin in peripheral blood increased in the RLS group compared with the healthy group,and there was a statistical difference between the two(p<0.05).4.In the period of migraine attack,the levels of calcitonin gene-related peptide and endothelin in peripheral blood were increased in non-RLS group compared with healthy group,but there was no significant difference between the two(p>0.05).5.Healthy group(r=0.536,p<0.05)and RLS group,non-RLS group migraine interval(r=0.706,p<0.05;r=0.495,p<0.05),calcitonin gene-related peptide and There was a positive correlation between changes in endothelin levels.in conclusion:1.This experiment suggests that changes in calcitonin gene-related peptide and endothelin levels are closely related to the onset of migraine.2.The right-to-left shunted peripheral blood calcitonin gene-related peptide and endothelin in patients with aura migraine increased in the migraine interval.3.Pearson correlation analysis suggests that there is a certain balance between peripheral blood calcitonin gene-related peptide and endothelin in healthy people and migraine attack.In the pathological situation of migraine attack,the two lose balance.
