Meta-analysis Of Oncologic Safety Of Autologous Fat Transplantation For Breast Reconstruction After Mastectomy

Objective:To evaluate the safety of this technique,by using evidence-based medicine,the oncology safety of breast reconstruction with autologous fat transplantation after mastectomy and without breast reconstruction after traditional mastectomy was analyzed including local recurrence,regional lymph node metastasis,and overall survival.Method:According to PRISMA guidelines,the main database in both English and Chinese,such as,PubMed,Embase,the Cochrane library,CNKI and Mainfest,has been carried on the literature search between January 2008 and December 2018,all with exposure to autologous fat transplantation?AFT?of patients with breast cancer research,including randomized controlled studies and nonrandomized controlled study.The experimental group for AFT breast reconstruction after resection of breast cancer patients.In the control group,simple breast cancer resection was performed without AFT;To avoid omission,references included in the study were manually searched.The main outcome of the meta-analysis was the difference in local tumor recurrence,regional lymph node and5-year overall survival rate.Result:A total of 7 studies^[1-7]were included,with a total of 5880 patients,all 7studies were cohort studies.The mean follow-up time was 4-5 years after mastectomy and 2-4years after autologous fat transplantation.The meta-analysis of the study showed that P?local recurrence?=0.654,P?regional lymph node metastasis?=0.644,P?5-year overall survival rate?=0.740 and there was no statistically significant difference between the autologous fat transplantation group and the simple surgical resection group in local tumor recurrence and regional lymph node metastasis.Conclusion:A meta-analysis of all the published literature on oncology data showed that after breast reconstruction with autologous fat transplantation,the local recurrence rate and regional lymph node metastasis rate were similar to that of traditional simple surgery,and autologous fat transplantation which is relatively safety do not lead to the increase of local recurrence rate and regional lymph node in breast cancer patients.
