Objective: To investigate the correlation between platelet-lymphocyte ratio(PLR)and severity of coronary lesions in elderly patients with acute coronary syndrome(ACS)and the predictive value of PLR for major adverse cardio cerebral vascular events(MACCE)after percutaneous coronary intervention(PCI).Methods: Three hundred and forty-four elderly patients who underwent PCI for 2-24 months and coronary angiography(CAG)were divided into MACCE group(n=95)and no-MACCE group(n=249)in our hospital from May 2016 to August 2018.30 healthy people in the same period were selected as control group.According to ROC curve,the selected patients were divided into high PLR group and low PLR group.According to the results of CAG,the patients were divided into single-vessel,double-vessel and three-vessel lesion groups.The patients were divided into low Gensini group?median Gensini group and high Gensini group.The clinical data of the selected patients were analyzed,the prognostic threshold of PLR was determined by ROC curve and logistic regression analysis was performed.Results:(1)LDL-C,TG,TC and PLR in ACS group were higher than those in control group(P<0.05),while HDL-C in control group was higher than that in ACS group(P<0.05).(2)The rates of smoking,diabetes mellitus,LDL-C,PLR and Gensini in MACCE group were higher than those in no-MACCE group(P<0.05).(3)The area under the curve of predicting poor prognosis of PCI in elderly ACS patients with PLR and Gensini score was 0.898(95% CI: 0.859-0.937)and 0.842(95%CI: 0.794-0.891).(4)PLR was positively correlated with Gensini score(r=0.654,P<0.05).(5)High PLR is an independent risk factor for predicting MACCE in elderly patients with ACS after PCI(OR=2.527,95%CI: 1.058-6.039,P=0.037).(6)Compared with non-three-vessel lesion group,PLR in three-vessel lesion group increased significantly(P<0.05);Compared with single vessel lesion group,PLR in double vessel lesion group increased significantly(P<0.05).(7)Compared with the low Gensini group and the median Gensini group,the percentage of patients with PLR was significantly increased in the high Gensini group(P<0.05);Compared with the low Gensini group,the above index was significantly increased in the median Gensini group(P<0.05).Conclusion: The level of PLR in peripheral blood is closely related to the incidence of ACS and the prognosis of PCI.
