Objective: This study was designed to compare the differences in functional damage between the near-triangle and near-dome detrusor of the bladder durings the progression of diabetic cystopathy.The previous study of the detrusor injury was introduced from the time series difference into spatial distribution differences.In clinical terms,it provides better location selection for bladder function repair of diabetic cystopathy patients,helping to achieve more accurate,more efficient,and more economical medical goals.Methods: Healthy female SD rats were selected,the diabetic group was given STZ for molding,and the control group was given sodium citrate buffer.Diabetic rats were divided into four groups: DM4 w,DM8w,DM12 w,and DM16 w.The experimental tests were performed at 4th week,8th week,12 th week,and 16 th week after modeling.Rats in the control group were tested at 4 weeks after modeling.The urodynamic index was used to evaluate the bladder dysfunction in rats.The mechanical stretch-stimulation test was performed on the near-triangle and near-dome detrusor ringss to evaluate the injury in different parts of the bladder.General information such as blood sugar,body weight and wet weight of the bladder were collected at the time of sacrifice.Results: 1.Compared with the Control group,the blood glucose level,weight loss,bladder wet weight increase,bladder wet weight and body weight ratio of the rats increased in each diabetic group.2.The maximum bladder volume,residual urine volume and detrusor leakage point pressure of the diabetic rats were higher than that of the Control group.3.The contractility of near-triangle detrusor rings in the DM4 w group and detrusor rings in the DM8 w group was higher than that in the Control group.The contractility of detrusor rings the DM12 w group and the DM16 w group was significantly lower than that in the Control group.4.In the control group,the near-triangle detrusor rings were stronger than the near-dome detrusor rings.But for the DM16 w group,the opposite is true.Conclusion: 1.The diabetic rats are reliable,and their bladder structure and function changes are exact and significant.2.The detrusor injury of diabetic rats was time-dependent.The contractility of the detrusor was compensatory in the early stage of diabetic cystopathy,but it decreased significantly in the middle-late stages of diabetic cystopathy.At the same time,the detrusor injury of diabetic rats was spatially different.The degree of contractility reduction between the near-triangle detrusor and the near-dome detrusor is not consistent.3.The bladder of normal rats has a positive contractility gradient from the upper part(the top of the bladder)to the lower part(the bladder outlet).Durings the progression of diabetic cystopathy,the original contractility gradient showed a deficit due to the differential injury between the near-triangle detrusor and the near-dome detrusor.This changed the mechanics of the bladder and reduced bladder emptying efficiency.This may be related to the urinary dysfunction characterized by "Excessive bladder volume and residual urine volume" in the late stage of diabetic cystopathy.
